摘  要:目的观察宫颈旁阻滞复合丙泊酚用于宫腔镜检查的效果。方法行宫腔镜检查的患者60例,随机分为观察组和对照组,手术开始前静脉注射丙泊酚至脑电双频指数(BIS)到60,观察组术前于左右宫颈旁各注射1%利多卡因5ml,对照组注射等剂量生理盐水。观察患者苏醒时间、丙泊酚用量,观察麻醉前(T1)、手术开始后5min(T2),患者苏醒后(T3)的心率(HR)、收缩压(SBP)和舒张压(DBP),术中低血压、心动过缓、低氧血症的发生率及术中体动次数。结果与观察组比较,对照组苏醒时间长,丙泊酚用量大,术中低血压、低氧血症发生率高,术中体动次数多(P<0.05),对照组T2时点SBP、DBP低于同组其他时点和观察组同时点(P<0.05)。结论利多卡因宫颈旁阻滞复合丙泊酚,可有效镇痛镇静,对呼吸循环干扰小,苏醒迅速。Objective To determine the effect of paracervical block associated with propofol for hysteroscopy.Meth-ods Sixty patients scheduled to undergo hysteroscopy were randomly assigned to two groups (observation and control group)with thirty each.All patients received intravenous propofol until bispectral index to 60 and paracervical block was administrated with 1% lidocaine 5 ml in observation group and saline in control group respectively before opera-tion.Awaking time of anesthesia and dosage of propofol were recorded.HR,SBP and DBP were determined before anes-thesia (T1 ),5 minutes after operation (T2 )and after patient awaking (T3 ).The incident of hypotension,bradycardia, hyoxemia and body movement during operation was also observed.Results Compared with observation group,there are longer awaking time,more dosage of propofol and higher incident of hypotension,hyoxemia and body movement in con-trol group (P 〈0.05).SBP and DBP were lower at T2 in control group than other stages in the same group and the same stage in observation group(P 〈0.05).Conclusion Paracervical block with lidocaine associated with propofol for hysteroscopy can get satisfactory analgesia,smooth respiration,stable haemodynamics and rapid recovery.
