摘 要:目的观察糖尿病慢性并发症患者血浆血栓调节蛋白(TM),蛋白C(PC),蛋白S(PS),抗凝血酶(AT-Ⅲ)的变化,了解这些因子在糖尿病慢性并发症凝血功能变化中的意义。方法按糖尿病常见并发症分为4组:糖尿病无并发症20例,糖尿病肾病(DN)20例,糖尿病视网膜病变(DR)20例,糖尿病足(DF)20例,以及年龄相匹配的健康正常人20例作为对照组,分别应ELISA方法测定血浆TM浓度,日本sysmex 6000全自动凝血分析仪测定PC,PS,AT-Ⅲ浓度。结果各血浆TM,PC,PS水平均明显高于正常对照组(P<0.05),AT-Ⅲ水平低于正常对照组(P<0.05);并发症组血浆TM,PC,PS水平均高于无并发症组(P<0.05),而AT-Ⅲ活性低于无并发症组(P<0.05),在有并发症组之间无显著变化(P>0.05)。结论糖尿病患者血浆TM,PC,PS及AT-Ⅲ的浓度变化对DN,DR和DF的早期诊断和病情分析有重要的临床价值。Objective In order to understand the significance of these factors in variation of coagulation of diabetic chronic complications,we observe the variation of plasma thromboxane regulating protein (TM),protein C (PC),pro-tein S (PS),antithrombin (AT-Ⅲ)in patients with chronic complications of diabetes.Methods The common complica-tion of diabetes were divided into 4 groups:diabetes without complications(20 cases),diabetic nephropathy (DN)(20 cases),diabetic retinopathy lesions (DR)(20 cases),diabetic foot (DF)(20 cases),as well as the normal healthy peo-ple with corresponding age(20 cases)as control groups,and the blood plasma TM concentration should be determined by ELISA,PC,PS,AT-Ⅲ concentration should be tested by Japan Sysmex 6000 fully automatic blood coagulation ana-lyzer respectively.Results The level of TM,PC and PS in blood plasma were significantly higher than control group (P〈0.05),AT-Ⅲ levels lower than the normal control group (P 〈0.05);the average level of TM,PC and PS of blood plasma in patients with complications was obviously higher than those patients without complications(P 〈0.05),how-ever,AT-Ⅲ activity of blood plasma in patients with complications was somehow lower than those patients without complications(P 〈0.05),which turned out to be original standard (P 〉0.05).Conclusion The variations of TM,PC, PS and AT-Ⅲ concentration in blood plasma to diabetic patient are clinically important for the early diagnosis and dis-ease analysis of DN,DR,and DF.
