摘 要:目的 评估食管癌患者术前营养状况及症状发生情况,分析两者之间的关系。方法 采用人体测量、实验室检查、营养风险筛查(NRS 2002)、主观全面评定法(SGA)以及记忆症状评估量表(MSAS)对75例食管癌患者进行测评。结果 42例(56.0%)患者存在营养风险;SGA评分存在营养不足16例(21.3%)。根据各项不同指标,营养不足检出率为5.3%~58.7%。患者平均经历6种症状。躯体症状评分与上臂围、上臂肌围呈负相关(均P〈0.05),与NRS 2002评分呈正相关(P〈0.05)。结论 食管癌患者术前常存在营养不足和营养风险,且伴有多种症状,症状与营养状况之间存在关联。建议在评估患者营养风险和制定营养支持策略时,结合患者的症状情况。Objective To describe nutritional status and symptoms in patients with esophageal carcinoma before surgery, and to analyze the associations between them. Methods A total of 75 patients with esophageal carcinoma were enrolled, and then they received assessments of nutritional status and symptoms before the surgery through physical examinations, laboratory examinations,and questionnaire survey with the Nutritional Risk Screening (NRS 2002), Subjective Global Assessment (SGA) and Memo rial Symptom Assessment Scale (MSAS). Results Forty-two (56. 0%) patients had nutritional risk, and 16 (21. 3%) patients presented nutritional deficiency evaluated by the SGA. Considering the different nutritional indexes, the prevalence of malnutrition ranged from 5.3 % to 58.7%. These patients experienced 6 symptoms. Score of physiological symptoms measured with the MSAS was negatively correlated with arm circumference and arm muscle circumference, whereas positively correlated with NRS 2002 score (P〈0.05 for all). Conclusion Patients with esophageal carcinoma suffer from malnutrition and nutritional risk before the surgery, and they experience several symptoms. The symptoms are associated with nutritional status. It is recommended that assess ment of nutritional status and provision of nutritional support should consider patients' symptoms.
