摘 要:目的探讨甲状腺、乳腺Ⅰ类手术患者围术期实施医院感染目标监测对降低抗菌药物使用的效果。方法分别选取2010年1-9月甲状腺、乳腺手术患者497例作为对照组,2010年10月-2013年11月511例为观察组,自2010年10月起由医院感染科与药学部联合对患者围术期抗菌药物使用进行调查,统计两组患者联合用药,并记录两组患者术前、术后用药时间以及感染发生率,数据采用SPSS13.0软件进行统计分析。结果观察组患者采取一联用药占88.65%,明显高于对照组59.36%,两组比较差异有统计学意义(P<0.05);观察组患者发生手术部位感染28例占5.48%,对照组为45例占9.05%,观察组明显少于对照组;所有观察组患者均在术前30min^2h使用抗菌药物,而对照组患者中术前用药时间>2h有83.50%;观察组患者术后用药时间≥3d占3.84%,明显小于对照组82.29%的比例,上述两组比较差异有统计学意义(P<0.05)。结论通过医院感染目标性监测可有效降低甲状腺、乳腺围术期抗菌药物的多联应用,缩短用药时间,并降低感染发生率,有效保证了抗菌药物合理应用。OBJECTIVE To investigate the effect of perioperative target monitoring for hospital infections on reducing antibacterial drug use in patients undergoing class I thyroid and breast surgeries.METHODS Totally 497 patients undergoing thyroid and breast surgeries during Jan.2010-Sep.2010 were enrolled as the control groups and 511 patients undergoing those surgeries during Oct.2010- Nov.2013 as the observation group.The investigation on the use of antibacterial drugs was carried out since Oct.2010 by the department of hospital infection and the department of pharmacy.The combination therapies in the two groups were summarized and preoperative and postoperative medication time and the incidence of infection were recorded.The software SPSS13.0was used for statistical analysis.RESULTS The patients taking single antibacterial usage accounted for88.65% in the observation group,significantly higher than 59.36% in the control group,the difference was significant(P 0.05).Surgical site infections occurred in 28 cases(5.48%)in the observation group,significantly less than 45 cases(9.05%)in the control group.All patients in the observation group had antibacterial drugs within 0.5h-2hbefore surgery,while 83.50% of the patients in the control group in 2hor longer before surgery.There were 3.84% of patients in the observation group who had antibacterial drugs at 3d or longer after surgery,significantly less than the proportion of 82.29% in the control group.The above difference was significant(P〈0.05).CONCLUSION The target monitoring for hospital infection can reduce perioperative multi-therapy of antibacterial drugs in thyroid and breast surgeries,shorten treatment time and reduce the incidence of infection,thus effectively ensuring the rational use of antibacterial drugs.
