老年心血管病患者肺部感染合并重度低钾血症的临床治疗研究

摘　　要：目的探讨老年心血管疾病住院患者肺部感染合并重度低钾血症的临床救治措施,以提高诊治水平。方法调查2011年1月-2014年1月的68例老年心血管疾病肺部感染合并重度低钾血症患者临床资料,将其随机分为对照组与观察组,每组各34例,观察组给予深静脉置管微量泵注射氯化钾溶液补钾治疗,对照组采用普通静脉滴注0.3%氯化钾溶液补钾治疗,比较两组患者治疗后的临床疗效、肺部感染改善及血钾水平。结果观察组治疗有效率为88.24%,显著高于对照组的67.65%,差异有统计学意义(P<0.05);观察组治疗前、后血钾水平分别为(2.18±0.13)、(3.98±0.42)mmol/L;对照组治疗前、后血钾水平分别为(2.46±0.15)、(3.51±0.38)mmol/L,显著高于对照组,差异有统计学意义(P<0.05)。结论深静脉置管补钾治疗和锁骨下静脉穿刺注射氯化钾溶液治疗,可提高老年心血管疾病住院患者肺部感染合并重度低钾血症的治疗有效率,提升患者生命质量,可于临床推广与应用。OBJECTIVE To investigate clinical treatment measures for pulmonary infections complicated with severe hypokalemia in elderly hospitalized patients with cardiovascular disease,in order to improve the diagnosis and treatment.METHODS Clinical data of 68 cases of pulmonary infections complicated with severe hypokalemia in elderly patients with cardiovascular disease were investigated and the patients were randomly divided into the control group and the observation group,34 cases in each group.The observation group was given potassium supplement by deep venous catheter for micro-pump injection of potassium chloride,while the control group was given common intravenous infusion of 0.3% potassium chloride solution for potassium supplement.The clinical efficacy,pulmonary infections and serum potassium level of the two groups were compared after treatment.RESULTS The effective rate was 88.24% in the observation group,significantly higher than 67.65% in the control group（P〈0.05）.The serum potassium levels before and after treatment were（2.18±0.13）mmol/L,（3.98±0.42）mmol/L in the observation group,significantly higher than（2.46±0.15）mmol/L,（3.51±0.38）mmol/L in the control group（P〈0.05）.CONCLUSION The treatment of deep venous catheter for potassium supplement and subclavian venous injection of potassium chloride solution can improve the treatment efficiency for pulmonary infection complicated with severe hypokalemia in elderly hospitalized patients with cardiovascular disease and improve the quality of life of patients,which is worthy of clinical promotion and application.
