反流性咽喉炎患者幽门螺杆菌感染的临床治疗研究

摘　　要：目的探讨分析反流性咽喉炎患者幽门螺杆菌(Hp)感染的临床治疗方案,以提高临床治疗效果。方法收集医院2012年1月-2013年12月收治的慢性咽喉炎患者216例,根据临床症状分为反流性咽喉炎组(观察组)136例和普通咽喉炎组(对照组)80例,两组患者均通过14 C尿素呼气试验筛查,根据结果不同采取相应的治疗,并对Hp阳性患者治疗后的Hp转阴率及临床疗效进行分析。结果观察组患者检出Hp阳性118例,阳性率86.8%,对照组患者检出Hp阳性53例,阳性率66.3%,两组比较差异有统计学意义(P<0.05);两组Hp感染患者经四联根除Hp治疗2周后,临床症状有明显好转,Hp转阴率均>80.0%,但两组总有效率比较差异无统计学意义。结论 Hp是慢性胃炎、消化性溃疡常见致病菌,与胃食管反流及反流性咽喉炎的发病有一定关系,对患者进行Hp筛查,进行分类治疗,是提高临床治疗的有效手段。OBJECTIVE To discuss and analyze clinical treatment of Helicobacter pylori（Hp）infections in patients with reflux laryngitis so as to improve clinical outcomes.METHODS Totally 216 patients with chronic laryngitis admitted during Jan.2012 to Dec.2013 were enrolled,and they were divided into the reflux laryngitis group（the observation group,136cases）and the common laryngitis group（the control group,80cases）according to the clinical symptoms.Patients were screened by 14 Curea breath test,and received appropriate treatment according to the test results.The HP negative conversion ratio and the clinical efficacy after treatment for Hp-positive patients were analyzed.RESULTS In the reflux laryngitis group,118 cases were detected as Hp positive,the positive rate was 86.8%;in the common laryngitis group,53 cases were detected as HP positive,the positive rate was 66.3%.The difference between the two groups was significant（P〈0.05）.Clinical symptoms were improved significantly for both groups after two weeks of quadruple Hp eradication therapy,the Hp negative conversion ratio was 〉80.0% for both groups,but the difference in total efficiency between the two groups was not significant.CONCLUSION Hp is the common pathogen for chronic gastritis and peptic ulcer,and has a certain relationship with the incidence of gastroesophageal reflux and reflux laryngitis.Hp screening of patients and classified treatment is effective to improve the diagnosis and treatment.
