宫颈上皮内瘤变患者高危型人乳头状瘤病毒感染LEEP术与保妇康栓联合应用的评价

摘　　要：目的对LEEP术与保妇康栓联合应用治疗宫颈上皮内瘤变(CIN)患者高危型人乳头状瘤病毒(HRHPV)感染的疗效进行观察和探讨,以期提升治疗水平。方法选择2012年2月-2013年8月在医院接受LEEP术治疗的CIN合并HR-HPV患者68例,按照随机数字法将患者分为观察组和对照组,各34例,观察组患者在LEEP术后加用保妇康栓;而对照组不进行特殊处理,对比两组患者术后阴道出血量、出血时间、排液量和排液时间、中医证候积分变化、HPV转阴率以及总疗效。结果观察组患者阴道出血量、出血时间、排液量和排液时间分别为(8.4±2.7)ml、(6.8±2.2)d、(10.2±3.6)ml和(7.5±2.5)d,对照组为(15.3±6.7)ml、(9.3±3.4)d、(16.2±5.4)ml和(9.7±3.4)d,两组相比,差异有统计学意义(P<0.05);观察组患者中医症状积分变化明显高于对照组,差异有统计学意义(P<0.05);观察组患者治疗总有效率及HPV转阴率分别为97.1%及79.4%,明显高于对照组76.5%及47.1%,差异有统计学意义(P<0.05)。结论 LEEP术联合保妇康栓治疗CIN合并HR-HPV具有显著的疗效,与单纯手术相比,患者症状改善更为明显,是科学且安全可靠的手术方式。OBJECTIVE To observe and analyze the effectiveness of LEEP surgery combined with Baofukang suppository for treatment of cervical intraepithelial neoplasia（CIN）complicated by high-risk human papillomavirus infections（HR-HPV）so as to improve treatment level.METHODS Totally 68 patients with CIN complicated by HR-HPV receiving LEEP surgery in the hospital from Feb.2012-Aug.2013 were selected.The patients were randomly divided into the observation group and the control group,each with 34 cases.Patients in the observation group were treated with Baofukang suppository after LEEP,while the control group was not given special treatment.Vaginal bleeding amount,bleeding time,draining amount,draining time,score changes of traditional Chinese medical（TCM）symptoms,HPV negative rate and total effective rate were recorded and compared.RESULTS The vaginal bleeding amount,bleeding time,draining amount and draining time was（8.4±2.7）ml,（6.8±2.2）d,（10.2±3.6）ml and（7.5±2.5）d respectively in the observation group,and was（15.3±6.7）ml,（9.3±3.4）d,（16.2±5.4）ml and（9.7±3.4）d in the control group,the difference was significant（P〈0.05）.The change of TCM symptom scores was significantly higher in the observation group than in the control group（P〈0.05）.The total effective rate and the HPV negative rate was 97.1% and 79.4%in the observation group,significantly higher than 76.5% and 47.1% in the control group,the difference was significant（P〈0.05）.CONCLUSION The LEEP surgery combined with Baofukang suppository for treatment of CIN complicated by HR-HPV has significant efficacy and the improvement of symptoms was more obvious compared with single surgery,therefore,this is a scientific and safe surgical method.
