股骨肿瘤假体表面涂布抗生素骨水泥降低术后感染的临床研究

摘　　要：目的探讨股骨远端骨肿瘤假体表面涂布抗生素骨水泥对术后假体周围感染的影响，以寻找最佳治疗方法，提高临床诊治水平。方法选取2010年4月-2013年4月70例股骨远端骨肿瘤患者为研究对象，将其分为对照组及观察组，每组各35例，对照组予以常规假体安装，观察组在对照组基础上在假体表面涂布抗生素骨水泥，观察两组治疗后临床效果差异性。结果对照组患者治疗优良率及总有效率分别为48．57％及82．86％，均低于观察组的74．29％及94．29％，两组比较差异有统计学意义（P〈O．05）；对照组患者再次感染率及并发症总发生率分别为11．43％及45．71％，均高于观察组的2．86％及8．58％，差异有统计学意义（P〈O．05）；对照组患者O～1年及〉3年生存率分别为28．58％及25．71％，与观察组的17．14％及45．72％比较，差异有统计学意义（P〈0．05）。结论假体表面涂布抗生素骨水泥治疗股骨远端骨肿瘤不仅降低了术后感染率，且促进了功能康复。OBJECTIVE To investigate the effect of the distal femur bone tumor prosthesis coated with antibiotic bone cement on infections around the prosthesis, so as to find the best treatment methods to improve clinical diagnosis and treatment. METHODS Totally 70 patients with distal femur bone tumor during Apr. 2010 to Apr. 2013 were selected as the research subjects and divided into two groups, the control group of 35 patients and the observation group of 35 patients. The control group was installed with conventional prosthesis and the observation group was installed with the prosthesis coated with antibiotic bone cement. The differences in clinical effect were observed for the two groups after treatment. RESULTS The excellent rate and the total efficiency was 48.57% and 82.86% in the control group, significantly lower than 74.29% and 94.29% in the observation group （P〈0.05）. The incidence of re-infection and the overall incidence of complications was 11.43% and 45.71% in the control group, significantly higher than 2.86 % and 8.58 % in the observation group （P〈0.05）. The 0-1 year survival rate and the over 3 year survival rate was 28.58% and 25.71% in the control group, significantly higher than 17.14% and 45.72% in the observation group （P 〈0.05）. on prosthesis for treatment of distal femoral bone tumor not CONCLUSION The coating of antibiotic bone cement only reduces postoperative infection but also promotes functional recovery.
