超声声脉冲辐射力成像技术在机械通气新生儿脑功能监测应用的研究

摘  要:目的:探讨应用超声声脉冲辐射力成像技术检测机械通气对新生儿颅脑功能的影响。方法:选择新生儿患儿共310例分为早产儿机械通气组40例及非机械通气组142例,足月儿机械通气组32例及非机械通气组96例。采用Siemens Acuson S2000超声诊断系统,生后4-7天常规行颅脑二维超声及彩色多普勒检查,测大脑中动脉RI值及左侧顶叶、右侧顶叶、左侧丘脑、右侧丘脑声触诊组织量化(VTQ)值,并对各组进行比较。结果:早产儿机械通气组大脑中动脉RI值明显低于非机械通气组,差异有统计学意义(P〈0.05);足月儿机械通气组双侧顶叶及左侧丘脑VTQ值〉非机械通气组,早产儿机械通气组双侧顶叶及左侧丘脑VTQ值〉非机械通气组,差异有统计学意义(P〈0.05);早产儿机械通气组双侧顶叶及左侧丘脑VTQ值〉足月儿机械通气组,差异有统计学意义(P〈0.05)。早产儿机械通气组中胎龄≤33周患儿双侧顶叶及左侧丘脑VTQ值〉胎龄〉33周患儿,差异有统计学意义(P〈0.05)。结论:在机械通气时与非机械通气时足月儿大脑中动脉RI值变化不大,早产儿则明显下降。双侧顶叶及左侧丘脑VTQ值机械通气组〉非机械通气,早产儿机械通气组变化更明显,胎龄越小变化越明显。Objective: To explore the effect of ultrasonic acoustic radiation force impulse (ARFI) imaging on mechanical ventilation neonatal brain function monitoring. Methods: A total of 310 neonates were divided into premature mechanical ventilation group (40 neonates), premature nonmechanical ventilation group (142 neonates), mature mechanical ventilation group (32 neonates) and mature non - mechanical ventilation group (96 neonates) . Siemens Acnson S2000 ultrasound system was used to conduct routine two - dimensional cerebral ultrasonography and color Doppler uhrasonography; resistance index (RI) of middle cerebral artery and virtual touch quantification (VTQ) values of left parietal lobe, right parietal lobe, left thalamus and right thalamus were detected, then the results in the four groups were compared. Results: RI of middle cerebral artery in premature mechanical ventilation group was statistically significantly lower than that in premature non - mechanical ventilation group ( P 〈 0.05 ) ; VTQ values of left parietal lobe, right parietal lobe and left thalamus in mature mechanical ventilation group were statistically significantly higher than those in mature non - mechanical ventilation group ( P 〈0. 05 ) ; VTQ values of left parietal lobe, right parietal lobe and left thalamus in premature mechanical ventilation group were statistically significantly higher than those in premature non - mechanical ventilation group ( P 〈 0. 05 ) ; VTQ values of left parietal lobe, right parietal lobe and left thalamus in premature mechanical ventilation group were statistically significantly higher than those in mature mechanical ventilation group ( P 〈 0. 05 ) ; in premature mechanical ventilation group, VTQ values of left parietal lobe, right parietal lobe and left thalamus in children ≤ 33 weeks were statistically significantly higher than those in children 〉 33 weeks (P 〈 0. 05 ) .
