柴胡注射液减轻人粒细胞集落刺激因子不良反应的临床观察

摘　　要：目的评价柴胡注射液(北柴胡)减轻乳腺癌患者化疗期间接受人粒细胞集落刺激因子(G-CSF)治疗时的不良反应。方法 150例需G-CSF支持的乳腺肿瘤患者随机分两组,柴胡注射液组77例,其中24例接受(多西他赛+阿霉素+环磷酰胺),26例接受(环磷酰胺+表阿霉素+5-氟尿嘧啶),27例接受(阿霉素+环磷酰胺)→(多西他赛);对照组为73例,接受心理护理或物理降温处理。柴胡注射液组在皮下注射G-CSF期间每日肌注4 m L柴胡注射液,每日1次。检测两组患者治疗前后血中性粒细胞,疼痛程度,体温及肝肾功能指标。结果两组患者治疗前后血中性粒细胞及肝肾功能指标差异无统计学意义。疼痛缓解有效率及体温变化差异,其差异有统计学意义(P<0.05)。结论柴胡注射液在不影响G-CSF治疗效果的前提下,能安全有效的减轻其骨痛、肌肉疼痛和发热的不良反应。AIM To evaluate the effect of Injectio Bupleuri（Bupleurum chinense） in counteracting the adverse reactions of G-CSF in patients＇ breast cancer treatment.METHODS One hundred and fifty cases of breast cancer victims needing G-CSF support were assigned randomly into trial group（among 77 cases,24 receiving docetaxel＋ doxorubicine ＋ cyclophosphamide; 26 getting cyclophosphamide ＋ epirubicin ＋ 5-fluorouracil; 27 having doxorubicine ＋ cyclophosphamide,were then succeeded by docetaxel）.Seventy-three cases were for psychological therapy and / or physical cooling treatment in the control group.Patients in the trial group were given Injection Bupleuri（4m L） once a day in the period of rh G-CSF（i.h.）.The clinic indexes,blood neutrophils,pain degree,body temperatures,the liver and kidney functions prior to and after the treatment were monitored.RESULTS The blood neutrophils and liver and kidney function of both groups＇ patients showed no significant difference before and after the treatment.But the extent of pain relief and the body temperature drop between the two groups were statistically significant（P〈0.01）.CONCLUSION Injectio Bupleuri can reduce bone and muscle pain,and lower fever safely and effectively with no impairment to the therapeutic effect of G-CSF.
