社区2型糖尿病合并失眠患者中医证型研究

摘　　要：目的:探讨社区2型糖尿病合并失眠患者的中医证型及与糖尿病并发症的相关性。方法:将120例2型糖尿病合并失眠患者按辨证分型标准进行辨证分型,同时筛选90例2型糖尿病未失眠患者作为对照组,对两组糖尿病患者进行筛查,分析其本虚证型的构成及证型分布与BMI指数、病程、年龄、糖尿病并发症的相关性。结果:糖尿病合并失眠患者按本虚证型分为气虚、血虚、阴虚、阳虚4型;其与BMI指数、年龄、病程均有相关性;糖尿病并发症筛查结果显示失眠本虚证型与糖尿病心血管病变和糖尿病脑血管病变(r=0.471,P<0.01)均相关,组间比较显示失眠患者在并发症发病率和多并发症共同发病例数均高于对照组,差异具有统计学意义(P<0.05)。结论:糖尿病合并失眠患者的中医本虚证型分布集中于阴虚证,糖尿病合并失眠及本虚证型分布与糖尿病并发症明显相关,提示生存质量和情绪变化导致的失眠可能对糖尿病并发症有一定的影响。Objective： To explore the correlation between TCM syndrome type and diabetes complications of patients with community type II diabetes combined with insomnia. Methods： 120 patients with type II diabetes combined with insomnia（ T2 DM ＋ I）were classified according to differentiation type standards. At the same time 90 patients with only type II diabetes were chosen into the control group. The diabetes complications of all the patients in both groups were screened; The correlation between formation and distribution of original deficiency syndrome type,and BMI index,disease course,age and diabetes complications were analyzed. Results： The types of patients with T2 DM ＋ I include qi deficiency,blood deficiency,yin deficiency and yang deficiency; There was correlation between original deficiency syndrome type and BMI index（ r =- 0. 189,P〈0. 05）,age（ r =0. 220,P〈0. 05）,disease course（ r = 0. 226,P〈0. 05） respectively; The screening result of diabetes complications indicated that there was correlation between insomnia original deficiency syndrome type and diabetic cardiovascular disease（ r = 0. 212,P〈0. 05） and the diabetic cerebrovascular disease（ r = 0. 471,P〈0. 01）; The complication reoccurrence rate and multi-complication reoccurrence rate of patients with insomnia in the treatment group were higher than those of the control group,and their difference had statistical meaning（ P〈0. 05）. Conclusion： The distribution of TCM original deficiency syndrome type of patients with T2 DM ＋ I focuses on yin deficiency syndrome; There is clear correlation between T2 DM ＋ I and original deficiency syndrome distribution and diabetes complication; Insomnia caused by living quality and mood changes probably has influence on diabetes complications.
