肺结核病人中丙型肝炎分布特征及其与肝功能损害的关联性分析

摘　　要：目的 了解肺结核病患者丙型肝炎（简称丙肝）的分布特征及其与肝功能损害之间的关系,为今后有效地开展肺结核病与丙肝的健康教育提供参考依据。方法 收集新疆部分地区肺结核患者的一般资料并采集血清进行检测。对其进行流行病学分析。结果 此次研究共调查2106例肺结核患者,其中男性1168例,女性938例;调查对象的年龄为（43.9±18.3）岁。肺结核病人中丙肝的总感染率为2.0%,男性肺结核患者的丙肝感染率（2.8%）高于女性（1.1%）（χ2=8.05,P=0.005）。肝功能损害发生率为4.7%。肺结核合并丙肝患者肝功能损害检出率（16.3%）高于未合并丙肝的肺结核患者（4.5%）（χ2=10.63,P=0.001）;控制其他可能的影响因素后,合并丙肝感染作为肝功能损害发生的危险因素,其OR（95%CI）为3.57（1.52～8.40）。结论 肺结核患者合并丙肝感染会大大增加肺结核人群肝功能损害发生的危险性,应加强对肺结核人群的预防保健措施,积极开展预防丙型肝炎病毒感染宣传教育。Objective To study the distributional characteristics of pulmonary tuberculosis patients with hepatitis C and the relationship between the liver function damage,for providing evidences for health education of tuberculosis and the hepatitis C. Methods General information of tuberculosis patients in parts of Xinjiang was collected and the serum samples were detected. The epidemiological analysis was used. Results A total of 2 106 cases of tuberculosis patients were investigated, including 1 168 cases of male and 938 cases of female. The average age of the subjects was （43.9 ± 18.3）. The infection rate of pulmonary tuberculosis in patients with hepatitis C was 2. 0% ; hepatitis C infection rate （2. 8% ） of male tuberculosis patients was higher than that of female （ 1.1% ）. The incidence of liver function damage was 4. 7%. Liver function damage detection rate （ 16. 3% ） in tuberculosis patients with hepatitis C was higher than that of tu- berculosis patients without hepatitis C （4. 5% ） , and the difference was statistically significant. When other possible influ- encing factors were brought under control, the concurrent hepatitis C infection was a risk factor for occurrence of liver func- tion damage, OR（95% CI） was 3.57 （1.52-8.40）. Conclusions Infection of tuberculosis in patients with hepatitis C significantly increase the risk of liver function damage. It is of great importance to take effective measures to prevent the transmission of Hepatitis C in patients with tuberculosis.
