摘 要:目的探讨煤工尘肺患者血清白介素18抗体(antibody against Interleukin 18,anti-IL-18)与类风湿因子(rheumatoid factor,RF)的表达水平及两者的相关性。方法采用间接酶联免疫吸附法(enzyme-linked-immunosorbent serologic assay,ELISA)对328例煤工尘肺患者、230例观察对象、309例接尘者及393例健康对照者血清IL-18抗体及RF的表达水平进行检测。结果病例组、观察对象组及接尘组人群血清IL-18抗体及RF的表达水平均高于健康对照组,差异有统计学意义(均有P<0.05);贰期煤工尘肺患者血清IL-18抗体的表达水平高于壹期尘肺患者(P=0.019),叁期尘肺与壹期尘肺相比患者血清IL-18抗体表达水平差异无统计学意义(P=0.067)。不同X线分期患者血清RF的表达差异无统计学意义(P=0.765);Spearman相关分析结果显示,各组中IL-18抗体与RF间均不存在相关性。结论煤工尘肺患者血清IL-18抗体及RF均存在高表达,且在一定范围内随X射线胸片表现分期的增高表达升高;但是两者间不存在相关性,提示IL-18抗体及RF可能通过不同的作用机制参与煤工尘肺发生发展。Objective To explore the level and correlation of antibodies against interleukin 18 ( anti - IL - 18 ) and rheumatoid factor(RF) in the sera of coal workers with pneumoconiosis (CWP). Methods A total of 328 patients with CWP, 230 observation subjects, 309 dust exposed workers and 393 normal controls were selected for the study. ELISA kit was used to detect anti-IL-18 and RF in the sera of all cases. Results Anti-IL-18 and RF were significantly higher in CWP group, observation subjects group and dust exposed group than those in control group ( all P 〈 0.05 ). The level of anti-IL-18 in stage U was significantly higher than that in stage I (P =0. 019), but showed no significant difference with stage HI ( P = 0. 067 ). No significant differences of RF expression were found among the three stages( P = 0. 765 ). The re- sults of spearman showed that the correlation between anti-IL-18 and RF in CWP group, observation subjects group, dust exposed group and control group were as follows : r = 0. 04,P = 0. 456 ; r = 0. 05, P = 0. 386 ; r = 0. 06, P = 0. 310 ; r = 0. 08, P = 0. 127, without correlation between anti-IL-18 and RF. Conclusions There were over-expressions of anti-IL-18 and RF in CWP but no correlation was found between them, suggesting a different mechanism in CWP development.
