胆囊切除术中胆囊动脉变异分析

摘　　要：目的 分析胆囊切除术中的动脉变异情况。方法 2010年1月～2013年12月灵宝市第一人民医院完成胆囊手术528例,异变28例,进行分析。结果 胆囊动脉变异临床常见,包括起源、分型、走行及进入胆囊部位等。起源变异：来自右肝动脉18例,胃十二指肠动脉8例,肝固有动脉2例。胆囊动脉分型及走行变异：主干单支型胆囊动脉10例;分支型胆囊动脉18例。胆囊动脉进入胆囊部位亦有胆囊管左侧、右侧,胆囊体多种进入方式。如不妥善处理变异胆囊的动脉会造成术中大出血,危及患者生命及造成胆总管损伤。结论 认识胆囊动脉变异的重要性,基层医院胆囊切除术中对胆囊动脉变异的重视及处理更为重要。Objective To analyze gallbladder artery variation in cholecystectomy. Methods From January 2010 to December 2013 in first people＇s hospital of Lingbao 528 cases of gallbladder surgery were completed. Variation of 28 cases were analyzed. Results The clinical common cystic artery variation included the origin, classification, and into the gallbladder parts, etc. The origin of variation： 18 eases originated from right hepatic artery, 6 cases from gastroduodenal artery, proper hepatic artery in 2 cases, 2 cases from gastroduodenal artery. Cystic artery type and line variation： main single type was in 10 cases of cystic artery; 18 cases were cystic artery branch type. Ways of cystic artery into the gallbladder parts also had in the left, right, into the gall bladder body. If not properly deal with the variation of gallbladder artery, it could cause intraoperative bleeding risk, endangered the patient＇s life and common bile duct damage. Conclusion We should know the importance of the cystic artery variation and pay more attention of the cystic artery variation and dispose is more important in cholecystectomy in basic-level hospitals.
