中医综合治疗对肺心病缓解期近期生存质量的临床观察

摘　　要：目的观察中医综合治疗方案对慢性肺心病缓解期近期生存质量的临床疗效。方法将181例肺心病缓解期患者随机分为观察组(中医综合治疗)和对照组(基础治疗)。辨证分型后,观察组在常规西医基础治疗的基础上,加用口服中药汤剂、穴位敷贴治疗;对照组给予西医基础治疗。治疗2个月后观察疗效、WHOQOL-BREF量表生存质量积分及各维度评分。结果总有效率观察组优于对照组,差异有显著统计学意义(P<0.01)。治疗后两组生存质量积分均明显改善,与治疗前比较差异有显著统计学意义(P<0.01),且观察组优于对照组(P<0.01)。两组在生存质量改善的各维度中,主要在生理领域、心理领域有明显的改善(P<0.01);而在社会领域、环境领域无明显改善(P>0.05)。结论中医综合方案和基础治疗方法对肺心病缓解期近期生存质量均有较好疗效,前者疗效优于后者。Objective To observe the clinical effect of comprehensive treatment of TCM on the life quality of patients with pulmonary heart disease in the remission stage. Methods 181 cases of patients with pulmonary heart disease in the remission stage were randomly divided into treatment group （TCM Comprehensive Therapy） and control group （basic treatment）. After the differentiation, the treatment group based on conventional western medicine basic treatment was combined with TCM decoction orally and acupoint application therapy. The control group was given the basic treatment of Western medicine. The curative effect, the index of WHOQOL-BREF and score of each dimension were observed after 2 months treatment. Results The total effective rate in the treatment group was better than the control group, there was significant difference （P〈 0.01）. The index of WHOQOL-BREF in two groups after treatment were significantly improved, the differences compared with those before treatment were statistical significant（P〈0.01）, and the observation group was superior to the control group （P〈0.01）. The physiological and psychological field of QOL in each dimension have been significantly improved （P〈0.05）, while there was no significant improvement in the social the environment field （P〉0.05）. Conclusion TCM Comprehensive regimen and foundation treatment method are effective in the quality of life of patients with pulmonary heart disease in the remission stage, but the effect of the former was superior to the latter.
