录像脑电图长程监测对发作性疾病的诊断价值

摘  要:目的:研究录像脑电图对发作性疾病的临床诊断和鉴别诊断的价值。方法:对148例临床发作性症状表现患者行录像脑电图长程监测,监测时间15-20h。结果:148例患者,录像脑电图异常者127例(85.81%),均在睡眠状态下出现痫性放电,其中仅38例(25.68%)伴有清醒状态下异常。睡眠异常多出现在NREMⅠ-Ⅱ期(90.55%),REM期未见。有97例(76.38%)睡眠异常患者,痫性波夹杂在睡眠纺锤中和/或其前后爆发。结论:录像脑电图监测对发作性疾病判定是否患有癫痫的诊断中起决定性作用;清醒状态下能检出痫性放电者,睡眠时就一定能够检测出来;睡眠纺锤对痫性放电可能有诱发和强化作用。Objective: Value of video-electroencephalogram(VEEG), which is usecl on clinical cuagnosm oi epilepsy, sort and differential diagnosis. Methods: Patients of one hundred and forty-eight who have clinic outbreak symptom were monitored with VEEG for a period,which lasted from fifteen to twenty hours. Results: One hundred and twenty-seven patients (rate:85.81% ) are abnormal, who are monitored with epilepsic EEG in sleepy time, and thirty-eight of it( rate: 25.68%) are abnormal in clear-headed time. Sleep abnormal EEG often were monitored in NREM I - U (rate: 90.55 % ), nothing is seen in REM. Ninty-seven patients(rate: 76. 38% ) whose epilesic wave break out in sleepic spin- dle period or around it. Conclusion, VEEG have a crucial function on clinical diagnosis of epilepsy, sort and differential diagnosis can check a xian to turn on electricty under the awake status, sleep can definitely examineSleepic spindle period can induce epilepsic wave and enhance it.
