短期使用米力农治疗难治性心衰的临床效果研究

摘　　要：目的研究短期使用米力农治疗难治性心衰的近期临床效果;方法选择120例难治性心衰患者,按照入院先后顺序编号,根据随机数字表法分成两组,每组60例,分别为对照组及研究组。其中对照组予以利尿剂、血管紧张素转换酶抑制药、硝酸酯类药物等常规治疗,而研究组在对照组治疗方案的基础上加用米力农,疗程均为7 d,比较治疗前后两组的相关指标及临床效果情况。结果在每搏输出量(CO)、左心室射血分数(LVEF)、左室收缩末期腔径(LVESD)、左室舒张末期腔径(LVEDD)、脑钠肽(BNP)、6 min步行距离方面,两组治疗后均有明显改善(P<0.05),而研究组的改善程度比对照组更显著(P<0.05);研究组的总有效率(73.33%)显著高于对照组(46.67%,P<0.05)。结论短期使用米力农能够显著改善难治性心衰患者的心功能及临床症状,而且具有较好的安全性。Objective To study short-term clinical effect of short-term use of milrinone in the treatment of refractory heart failure. Methods Total 120 hospitalized patients with refractory heart failure were randomly divided into two groups（ the control group and research group ; n = 60, respectively） by digital method according to the order of admission number. Routine diuretics, angiotensin- converting enzyme inhibitor, and nitrates were administered in the control group, on basis of the control group, milrinone was added in the research group. The courses of treatment in both groups were seven days. And then, the relevant indicators and clinical effects between the two groups before and after the treatment were compared and analyzed. Results The cardiac output（CO） ,left ventricular ejection fraction （ LVEF）, left ventricular end systolic diameter （ LVESD ）, left ventricular end diastolic diameter （ LVEDD ）, brain natriuretic peptide（BNP） ,6-minutes walking distance in both groups were improved significantly after the treatment（ P 〈 0. 05 ）, and the improvement in the research group was more significant than that in the control group （ P 〈 0.05 ） ; the total effective rate in the research group was 73.33% and that in in the control group was 46. 67%, with a statistically significant difference between the two groups（ P 〈 0. 05 ）. Conclusion The short-term use of milrinone can improve the cardiac fimction and clinical symptoms of the patients with refractory heart failure, and has high security.
