锁定钢板内固定联合抗骨质疏松药物治疗老年肱骨近端骨折的临床效果观察

摘　　要：目的探讨锁定钢板内固定联合抗骨质疏松药物治疗老年肱骨近端骨折的效果。方法将我院2011年2月至2013年10月收治的62例老年肱骨近端骨折患者随机分为观察组30例和对照组32例。两组均给予锁定钢板内固定治疗，观察组术后给予抗骨质疏松治疗。观察并比较两组患者的住院时间、治疗效果及骨密度。结果观察组和对照组住院时间分别为（6．9±1．0）、（9．5±1．1）d，两组比较差异有统计学意义（t=8．426，P〈0．05）。观察组的术后肩关节功能评分为优17例、良10例、可2例、差1例，对照组为优14例、良10例、可5例、差3例，两组比较差异有统计学意义（x2=4．617，P〈0．05）。抗骨质疏松治疗后，观察组骨密度为（O．79±0．13）g／cm2、对照组为（0．66±0．05）g/cm2，两组比较差异有统计学意义（t=10．251，P〈0．05）。结论锁定钢板内固定联合抗骨质疏松药治疗老年肱骨近端骨折效果较好，安全可靠，值得临床推广应用。Objective To investigate the effecof the locking plate fixation with anti-osteoporosidrug in the treatmenof oldepatientwith proximal humeral fractures. MethodSixty-two patientwith proximalhumeral fracturewho were treated in Yuzhou Mining Company Hospital of Kailuan Group of Hebei Province from Feb. 2011 to Oct. 2013 were divided into observation group（ n = 30） and control group （n = 32）. Patientin both groupwere given locking plate fixation, and patientin the observegroup were also received anti- osteoporositreatment. The information including hospitalization periods, efficacy and bone mineral density of the two groupwarecorded. ResultThe hospitalization periodof the observation group and control group were （6. 9± 1.0） d, （9. 5± 1.1 ） d, and the difference wastatistically significan（ = 8.426, P 〈 0. 05 ）. The efficacy in observegroup waexcellenin 17 cases, good in 10 Cases,faiin 2 caseand pooin 1 case, and the efficacy of the control group waexcellenin 14 cases, good in 10 cases, faiin 5 caseand pooin 3 cases. The difference wastatistically significan（ x2 = 4. 617, P 〈 0. 05 ）. Aftethe anti-osteoporositreatment, the bone mineral density of the observegroup and control group were （ 0. 79 ±0. 13） g/cm2 and （0. 66 ±0. 05） g/cm, the difference wastatistically significan（ = 10. 251, P 〈 0. 05 ）. Conclusion The effecof locking plate fixation with anti-osteoporosidrug ibettewith shortehospital stay, safe and reliable, and iiworthy of clinical application.
