摘  要:目的观察2型糖尿病伴抑郁患者血清C反应蛋白(CRP)和同型半胱氨酸(Hcy)水平的变化,探讨其临床意义及可能机制。方法选取124例2型糖尿病患者根据抑郁自评量表(SDS)分为抑郁组63例和非抑郁组61例。采用焦虑自评量表(SAS)进行评分验证,比较两组性别、年龄、受教育时间、体质量指数、病程、合并症个数、CRP、Hcy、空腹血糖(FPG)、糖化血红蛋白(HbAlc)和血脂水平。同时又将抑郁患者分为轻、中、重3组,比较Hcy、CRP的水平变化。结果抑郁组Hcy[11.5(8.6,15.6)μmol/L]、HbAlc[(10.13±2.17)%]、合并症个数[2(1,3)]高于非抑郁组[8.6(7.4,11.2)μmol/L、(9.33±2.20)%、1(0,2)];受教育时间[(9.75±3.36)年]低于非抑郁组[(11.56±3.73)年],差异均有统计学意义(t或z值分别为-3.537、0.952、-2.339、0.228,P均〈0.05)。抑郁患者轻、中、重3组间Hcy[8.75(7.45,10.45)、12.20(8.90,14.40)、19.50(14.33,28.03)μmol/L]差异有统计学意义(F:25.963,P=0.000),CRP[2.35(1.10,4.92)、3.25(1.11,5.68)、2.32(1.27,5.41)mg/L]及其他指标差异无统计学意义(F=0.194,P=0.907);抑郁分数与Hcy呈显著正相关(r=0.615,P=0.000)。结论2型糖尿病伴抑郁与血糖平均水平、受教育时间及病程有关;Hcy是2型糖尿病伴抑郁的独立危险因素,而CRP可能不是。Objective To investigate the changeof C- reactive protein (CRP) and homocysteine (Hcy) in the type 2 diabetewith depression, and itclinical significance and potential mechanism. MethodOne hundred and twenty-foucasewith type 2 diabetewere divided into the depression group (63 cases) and non-depression group (61 cases) according to the Self-Rating Depression Scale and verified by Self-Rating Anxiety Scale. The information including age, sex, education degree, body masindex, course of disease and the numbeof complicationwere recorded. The levelof CRP, Hcy, fasting plasmglucose ( FPG), glycosylated hemoglobin(HbAlc) and blood lipid were measured. The depression group wadivided into mild, medium and heavy group to compared the changeof Hcy and CRP. ResultThe levelof Hcy, HbAland the numbeof complicationin depression group were 11.5 (8.6, 15.6)μmol/L, ( 10. 13 ± 2. 17)%, and 2 ( 1,3 ) respectively, highethan thaof non-depression group ( 8.6 (7.4,11.2) μmol/L, (9. 33 ± 2.20) %, 1 (0,2) ), while the education degree of depression group ( (9.75 ± 3.36) years) walowethan thaof non-depression group ( ( 11.56 ± 3.73 ) years), and the differencewere significan( t/Z = - 3.537,0. 952, - 2. 339,0. 228 respectively;P 〈0. 05).
