骨皮质剥脱术联合锁定接骨板内固定术治疗四肢长骨骨不连的效果和安全性

摘　　要：目的探讨骨皮质剥脱术与锁定接骨板内固定联合治疗对四肢长骨骨不连患者的效果及安全性。方法2010年6月至2013年6月收治80例四肢长骨骨不连患者，按照治疗方式不同分为两组，每组各40例。观察组采用骨皮质剥脱术与锁定接骨板内固定联合治疗，对照组采用锁定接骨板联合开槽植骨法治疗。比较两组临床疗效和安全性。结果观察组治疗优良率[100％（40／40）]显著高于对照组[90．0％（36／40）]，差异有统计学意义（x2=4．21，P〈0．05）。术中出血量（370．5±56．5）ml显著低于对照组（486．5±64．5）ml，差异有统计学意义（t=8．56，P〈0．01）；骨折愈合时间（5．6±1．2）个月也显著低于对照组（7．2±1．5）个月，差异有统计学意义（t=5．27，P〈0．01）；术后并发症发生率5．0％（2／40）显著低于对照组20．0％（8／40），差异也有统计学意义（x2=4．11，P〈0．05）。结论骨皮质剥脱术与锁定接骨板内固定联合治疗对四肢长骨骨不连患者安全有效，能够减少术中伤害，加速术后骨折愈合。Objective To analyze the clinical effecand safety of cortical bone decortication combined locking plate fixation fotreatmenof long bone nonunion of limbs. MethodEighty patientwith long bone nonunion of limbwho were treated in the Zhongshan Hospital Affiliated to Dalian University from Jun. 2010 to Jun. 2013 were selected aousubjectand theiclinical datwere recorded. According to differentreatmenmethods, the patientwere divided into control group and observation group, and each group with 40 cases. Patientin observation group were treated with cortical bone decortication combined locking plate fixation, while in control group were treated with locking plate combined slotted bone graft. Clinical effectand safety of two groupwere compared. ResultExcellenand good rate in observation group wa100% （40/40） ,obviously highethan thaof control group （90. 0% （36/40） ;X2 =4. 21 ,P 〈0. 05）. Bleeding amounduring operation in observation group wa（ 370. 5± 56. 5 ） ml, obviously lesthan thaof control group （ （486. 5 ± 64. 5 ） ml ; = 8.56,P 〈 0. 01 ）. And the fracture healing time wa（ 5. 6 ± 1.2 ） monthin observation group, shortethan control group （ （ 7.2 ± 1.5 ） month; = 5.27, P 〈 0. 01 ）. The postoperative complication occurrence rate wa5. 0% （ 2/40 ）, significantly lowethan control group （ 20. 0% （ 8/40 ） ; X2 = 4. 11, P 〈 0.05 ）. Conclusion Cortical bone decortication combined locking plate fixation fotreatmenof long bone nonunion of limbisafe and effective,and ican reduce operation injury and accelerate postoperative healing of fracture.
