中老年人股骨转子间骨折术后并发证原因探析

摘　　要：目的：探析股骨转子间骨折术后相关并发证的原因,提出积极的防治方法。方法：88例患者中转子间稳定性骨折患者行动力髋螺钉固定（dynamic hip screw,DHS）术、不稳定性骨折患者行股骨近端髓内钉（proximal femoral nail,PFN）术;术后常规进行深部静脉血检（deep venous thrombosis,DVT）的预防、骨质疏松的治疗及个体化功能康复锻炼指导。结果：88例患者中80例获得随访,时间为6～12个月,未发生心、脑、肺、肾并发证、无感染病例、无DVT病例;手术并发证4例,占4.50%,技术缺陷2例,占2.30%,死亡1例,占1.40%。结论：严格把握手术适应证、调适内科疾患,准确复位、合理选择并妥善运用内固定,预防术后DVT、骨质疏松的积极诊疗与个体化的功能康复锻炼是减少术后并发证的关键。Objective： To analyze the reason of postoperative complications of senile intertrochanteric fracture and put forward active prevention methods. Methods： In 88 cases： intertrochanteric fracture stability dynamic hip screw（ DHS）,unstable fracture of proximal femoral nail（ PFN）; after conventional deep venous thrombosis（ DVT） prevention,treatment of osteoporosis and individualized rehabilitation exercise guide. Results： Among 88 cases,80 cases were followed-up,the time for 6-12 months,an average of 8 months,without the occurrence of cardiovascular and pulmonary and renal complications,no infection,no cases of DVT; operation complication 4 cases,accounting for 4. 50%,technical defects in 2 cases,accounting for 2. 30%,1 case died,1. 40%. Conclusion： Preoperative operation indication strictly,internal disease active stabilization,intraoperative fracture accurate reduction,internal fixation choice and accurate fixation,postoperative DVT active prevention,diagnosis and treatment of osteoporosis with positive individual functional rehabilitation exercise is the key to reducing postoperative complications.
