椎间盘镜下手术治疗高位腰椎间盘突出症的效果

摘　　要：目的：探讨高位腰椎间盘突出症在椎间盘镜（micro-endoscopic discectomy，MED）下手术治疗方法的效果。方法对10例高位腰椎间盘突出症患者采用MED进行手术。结果10例均成功完成MED下微创手术，手术时间1.0～1.5 h，出血量在10～50 ml，切口均一期愈合，无一例发生感染，住院时间5～7 d，平均6 d，术后当天腰腿疼痛症状明显缓解，全部获得随访，随访时间1～4年。结论 MED手术治疗高位腰椎间盘突出症具有创伤小、操作简单、手术安全性好、术后恢复快等优点。Objective To explore the diagnosis and operating methods of the high level lumbar disc herniation by micro-endoscopic discectomy（MED）.Methods 10 cases were all operated by MED successfully. Results The operating time was 1.0~1.5 hours, and the blood loss was 10~50 ml. The incision was primarily healed without any infection. The hospitalization was 5~7 days （mean 6 days）.Both the backaches and the leg pains were obviously reduced immediately after the operation. All the cases were followed up for 1~4 years. Conclusions Because of the defferent anatomic characteristics, the high level disc herniation had complex clinical manifestation. Therefore, the missed diagnosis rates and misdiagnosis rates were high, and the operating treatment was difficult. The methods of treating high level herniation by MED have the advantages of little trauma, simple, safe and rapid recovery.
