盐酸氢吗啡酮用于子宫全切术后静脉自控镇痛的效果

摘　　要：目的:观察不同剂量盐酸氢吗啡酮术后静脉自控镇痛的效果。方法:选取我院行子宫全切术患者150例,随机分为对照组、观察组Ⅰ和观察组Ⅱ各50例。对照组采用舒芬太尼1μg/ml、观察组Ⅰ采用盐酸氢吗啡酮0.04 mg/ml,观察组Ⅱ采用盐酸氢吗啡酮0.08 mg/ml行术后静脉自控镇痛,总量50 ml,并加入昂丹司琼8 mg。记录各组患者术后24 h自控镇痛泵(PCA)按压次数、各时间点Ramsay镇静评分和VAS镇痛评分、术后排气时间。记录过度镇静、呼吸抑制、恶心、呕吐、皮肤瘙痒、尿潴留等不良反应发生情况。结果:对照组和观察组Ⅱ术后24 h PCA按压次数和VAS评分明显低于观察组Ⅰ,Ramsay镇静评分高于观察组Ⅰ。结论:0.08 mg/ml盐酸氢吗啡酮可安全有效地用于子宫全切患者术后镇痛。Objective： To observe the effect of hydromorphone hydrochloride on patient-controlled intravenous analgesia （PCIA） after total hysterectomy. Methods： One hundred and fifty cases of patients with total hysterectomy were divided into a control group, an observation group I and observation group I1 with 50 cases each. Patients were treated with 1 gg/ml sulfentanil in the control group, and 0.04 and 0.08 mg/ml hydromorphone hydrochloride in the observation group I and II with total volume 50 ml, respectively. In addition, ondansetron （8 rag） was given as well. The effective pressing number, VAS score and Ramsay score and the time of postoperative exhaust in 24 h were recorded. Adverse reactions such as excessive sedation, respiratory depression, nausea and vomiting, pruritus and uroschesis were observed. Results： The effective pressing number and VAS score in the control group and observation group II were significantly lower than those in the observation group I, while the Ramsay score were higher. Conclusion： The application of 0.08 mg/ml hydromorphone hydrochloride in PCIA after total hysterectomy is safe and effective.
