摘  要:目的:探讨心力衰竭心肌微血管损伤与神经内分泌之间的相关性及通络干预作用。方法:胸主动脉缩窄法建立慢性心力衰竭模型,同期手术而不缩窄者为假手术组,设立卡托普利组、芪苈强心高中低剂量组,检测N端脑钠肽前体(NT-pro BNP)、血管紧张素Ⅱ(AngⅡ)、醛固酮(ALD)含量与去甲肾上腺素(NE)、血管性血友病因子(v WF)水平,观察心肌组织微血管内皮超微结构以及应用免疫荧光法检测心肌微血管密度。结果:各给药组不同程度降低神经内分泌因子含量,尤其芪苈强心高剂量组效果显著(P<0.01);透射电镜结果显示芪苈强心可改善微血管结构、减轻内皮损伤,CD34免疫荧光结果表明心肌组织微血管数量增加,同时血清ET-1、v WF的含量降低反映了其具有改善内皮功能的作用。应用相关分析法检测反映微血管内皮损伤的因素CD34、ET-1与神经内分泌因子之间的相关性,显示内皮损伤与神经内分泌因子水平呈正相关关系(P<0.05,P<0.01)。结论:芪苈强心有效保护微血管内皮,同时抑制神经内分泌过度激活,验证了微血管损伤与神经内分泌功能的过度激活相互作用、相互影响,为揭示脉络学说营卫"由络以通、交会生化"理论的科学内涵提供了实验依据。Objective: To study the correlation analysis of endothelial damage and neuroendocrine factors in rats with chronic heart failure and the intervention of regulating meridians. Methods: The rat model of chronic heart failure was established with thoracic aorta constriction(TAC) method. Rats operated with the thoracic aorta isolation but without constriction made up the sham group. The captopril group, Qili Qiangxin high, middle and low-dose groups were allocated. The contents of pro-N terminal brain natriuretic peptide(N-pro BNP), angiotensinⅡ(AngⅡ), and aldosterone(ALD) in rat serum were detected by radioimmunoassay. The levels of norepinephrine(NE) and v WF were detected by ELISA. The ultrastructure of microvessel endothelial cells in myocardial tissue was observed by transmission electron microscopy. The immunofluorescence technique was used to observe the microvessel density of myocardial tissue. Results: In the treatment groups, the levels of neuroendocrine factors and v WF were decreased in various degrees, the microvessel structure and endothelial function were improved, and the numbers of microvessel were increased. The correlation analysis showed that there was a positive correlation between the endothelial damage and neuroendocrine factors(P〈0.05, P〈0.01). Conclusion: Qili Qiangxin Capsule can effectively protect the microvascular endothelium, and suppress excessive activation of neuroendocrine factors. The interaction between the microvessel damage and excessive activation of neuroendocrine function are verified. These data can provide scientific supports for Ying-nutrient and Weidefense ‘unblocking collaterals with subsequent convergence and substance-qi transformation' in the vessel-collateral theory.
