Livin蛋白在胃癌中表达情况与中医证型相关性研究

摘　　要：目的:探讨Livin蛋白在不同病理类型胃黏膜中的表达,分析其表达与胃癌发生及与胃癌生物学特征的关系及与胃癌患者中医证型相关性。方法:纳入慢性胃炎、胃溃疡及胃癌患者151例,采用免疫组化SP法检测各中不同病变组中Livin蛋白的表达;四诊合参对胃癌患者进行中医辨证分型,对不同证型Livin蛋白表达阳性率进行分析。结果:1慢性胃炎组、胃溃疡组、胃癌组Livin蛋白表达阳性率分别为0.00%、8.33%、85.19%,胃癌组Livin阳性率明显高于其余各组(P<0.01);2低-未分化程度Livin蛋白表达高于高-中分化,印戒细胞癌组表达高于腺癌组,差异均有统计学意义(P<0.05);3胃癌患者中医证型以气血两虚证为主,该证型Livin阳性率明显高于其它证型(P<0.01)。结论:Livin蛋白与胃癌的发生与发展具有相关性;大部分胃癌患者存在气血两虚的情况,气血两虚可能患者体内Livin表达异常,导致胃癌的发生。Objective： Investigation of the expression of Livin protein in different pathological type of gastric mucosa to analyze the correlation between the expression of Livin protein and the gastric carcinoma onset, the biological characteristics of gastric carcinoma and the TCM syndrome. Methods： Patients with chronic gastritis, gastrelceosis and gastric carcinoma, 151 cases in total, were included. The immunohistochemistry SP method was used to detect the expression of Livin protein in different pathologic changes. The deduction by the four diagnostic techniques was used to differentiate the TCM syndrome types of patients with gastric carcinoma. The positive rate of the expression of Livin protein in different TCM syndrome types was analyzed. Results： 1The positive rate of the expression of Livin protein in the chronic gastritis group, the gastrelceosis group and the gastric carcinoma group was, in order, 0.00%, 8.33% and 85.19%, with the positive rate of the expression of Livin protein in the gastric carcinoma group being significantly higher than that in other groups（P〈0.01）. 2The expression of Livin protein in the low-no differentiation group was higher than that in the high-middle differentiation group, with statistical significances（P〈0.05）. The expression of Livin protein in signet-ring cell carcinoma group was higher than that in the adenocarcinoma group, with statistical significances（P〈0.05）. 3The TCM syndrome type of patients with gastric carcinoma was in a primarily qi-blood deficiency syndrome type, in which the expression of Livin protein was significantly higher than that in other syndrome types（P〈0.01）. Conclusion： There is a correlation between the expression of Livin protein and the onset of gastric carcinoma. Most patients with gastric carcinoma are differentiated into the qi-Blood deficiency syndrome type, for which the deficiency of qi and blood may be related to the disorder of Livin protein that may lead to the onset of gastric carcinoma.
