人工半肩关节置换治疗老年肱骨近端粉碎性骨折的临床分析

摘　　要：目的探讨分析老年肱骨近端粉碎性骨折患者行人工半肩关节置换术的临床效果。方法选择该院2010年1月—2012年6月收治的80例老年肱骨近端粉碎性骨折患者的临床资料进行回顾性整理,根据手术方式分为观察组和对照组两组,每组40例,观察组行人工半肩关节置换术,对照组行钢板内固定术,比较两组治疗效果。结果观察组手术时间、术中出血量及术后引流量明显少于对照组;观察组术后并发症发生率7.50%,显著低于对照组21.50%;观察组优良率92.50%,显著高于对照组72.50%,差异有统计学意义(P<0.05)。结论人工半肩关节置换术治疗老年肱骨近端粉碎性骨折,患者术后恢复快,并发症少,可以显著提高治疗效果。Objective To explore and analyze the clinical effect of artificial semi-shoulder joint replacement on comminuted frac-tures of the proximal humerus in the elderly. Methods The clinical data of 80 elderly patients with comminuted fractures of the proximal humerus admitted in our hospital from January 2010 to June 2012 were selected and sorted out retrospectively. According to the surgical mode, the patients were divided into two groups, the observation group and the control group with 40 patients in each. The observation group was treated by artificial semi-shoulder joint replacement, and the control group was treated by inter-nal fixation with steel plate. And the curative effects were compared between the two groups. Results The operation time, amount of bleeding during operation and postoperative drainage volume in the observation group were less than those in the control group, the differences were statistically significant （P〈0.05）. The incidence of postoperative complications of the observation group was 7.50%, it was significantly lower than that of the control group （21.50%）, the difference was statistically significant （ P〈0.05）. The excellent and good rate in the observation group was 92.50%, it was significantly higher than that of the control group （72.50%）, the difference was statistically significant （P〈0.05）. Conclusion For elderly patients with comminuted fractures of the proximal humerus, artificial semi-shoulder joint replacement can significantly improve the therapeutic effect, the patients can recover more quickly after the operation with less complications.
