H型高血压患者血小板活性的临床研究

摘　　要：目的探讨H型高血压患者血小板活性的变化及临床意义。方法选择原发性高血压患者169例,根据血浆同型半胱氨酸水平分为H型高血压组86例和对照组83例,分析2组24h动态血压均值、血栓素B2、血小板颗粒膜蛋白-140、血小板活化因子CD62p活性、血小板最大聚集率和同型半胱氨酸的差异,并进行比较。结果 2组患者24h动态血压均值收缩压、舒张压比较,差异无统计学意义(P>0.05)。与对照组比较,H型高血压组同型半胱氨酸、血栓素B2、血小板颗粒膜蛋白-140、血小板活化因子CD62p活性和血小板最大聚集率明显升高,差异有统计学意义[(25.36±0.93)μmol/L vs(14.34±0.75)μmol/L,(139.33±10.56)ng/L vs(110.76±12.91)ng/L,(30.55±3.52)μg/L vs(27.45±3.44)μg/L,(11.73±5.42)%vs(7.43±4.91)%,(43.32±4.51)%vs(35.24±4.42)%,P<0.05,P<0.01]。结论 H型高血压患者血小板活性明显升高,可能与H型高血压患者易出现心脑血管事件有关,应采取措施积极干预。Objective To study the change of platelet activity and its clinical significance in H type hypertension patients. Methods One hundred and sixty-nine essential hypertension patients were divided into H type hypertension group （n=86） and control group （n=83） according to their set um homocysteine levels. Their mean 24 h ambulatory blood pressure, serum thromboxane Be and homocysteine level, GMP 140 and CD62p activity, maximal platelet aggregation rate were analyzed,respectively. Results No significant difference was found in the mean 24 h ambulatory systolic and diastolic blood pressure between the two groups （P〉0.05）. The serum homocysteine and thromboxane B2 level, GMP-140 and CD62p activity, and maximal platelet aggregation rate were significantly higher in H type hypertension group than in control group （25.36±0.93 μmol/ L v.s 14.34±0.75 μmol/L, 139. 33 ±10. 56 ng/L w 110.76±12. 91 ng/L,30. 55±3. 52μg/L vs 27.45±3.44 μg/L,11. 73±5.42% vs 7.43±4.91%,43.32%±4.51%vs 35.24%±4.42%, P〈0.05,P〈0.01）. Conclusion The platelet activity is significantly higher in H type hypertension patients,which is associated with their cardio cerebrovascular events and should be controlled with active measures.
