摘 要:目的探讨小儿肺炎合并心力衰竭患者血浆脑钠肽(BNP)测定在鉴别诊断和疗效评估中的价值。方法选择2012年8月至2014年2月入住我院的80例1-3岁肺炎患者,其中单纯肺炎病例46例和肺炎合并心衰病例34例。每位患者分别在入组时和治愈出院前进行血浆BNP的测定,另取20名健康体检同年龄段儿童测定血浆BNP作为健康对照。将检测结果进行入组时单纯肺炎组与肺炎合并心衰组比较、肺炎合并心衰组治疗前后比较。结果肺炎合并心衰组入院时的血浆BNP(750±120 pg/ml)高于单纯肺炎组(135±50 pg/ml),差异有统计学意义(P<0.05)。肺炎合并心衰组患者治愈出院时的血浆BNP(85±45pg/ml)低于入组时(750±120 pg/ml),差异也有统计学意义(P<0.05)。结论血浆BNP是小儿肺炎合并心力衰竭患者临床诊断及监测疗效的一个敏感性指标,对于临床诊疗具有重要应用价值。Objective To investigate the differential diagnostic and therapeutic values of plasma brain natri-uretic peptide ( BNP) assay in children′s pneumonia complicated with heart failure. Methods The clinical data of 80 1-3 years old children with pneumonia were analyzed. They were divided into two groups, the simple pneumonia group (46 cases) and the pneumonia complicated with heart failure group (34 cases). All patients underwent twice plasma BNP assay:first on admission into hospital and second before discharge. Besides, 20 healthy children′s plas-ma BNP level were also detected as the control group. The plasma level of BNP was compared between the two groups. Results The plasma level of BNP was higher in the pneumonia complicated with heart failure group than that in the simple pneumonia group on admission (750 ± 120 vs 135 ± 50 pg/ml) (P〈0. 05). In the pneumonia ac-companied with heart failure group, the plasma level of BNP was higher when they were admitted to hospital than dis-charged (750 ± 120 vs 85 ± 45 pg/ml) (P〈0. 05). Conclusion Plasma BNP is a sensitive diagnostic and thera-peutic evaluative marker for children with pneumonia accompanied with heart failure, which is important to clinical diagnosis and treatment.
