多普勒超声显像对心力衰竭病人心脏同步化治疗评价

摘　　要：目的探讨多普勒超声显像对慢性心力衰竭（CHF）病人双心室再同步化治疗效果评价价值。方法选择慢性心力衰竭合并室内阻滞行双心室再同步化治疗的病人34例,采用多普勒超声心动图观察病人术前、术后1个月（参数优化前）、术后3个月及术后1年的心房前后径（LAD）、左心室舒张末内径（LVEDD）、左心室收缩末内径（LVESD）、左心室短轴缩短率（LVFS）及左心室排血分数（LVEF）;同时测量室间隔与左心室后壁间收缩延迟时间（SPWMD）、左心室舒张期充盈时间（LVFT）/R-R间期比值。结果双心室再同步化治疗术后1个月、3个月及1年各项指标较术前均明显改善,差异有显著性（F=19.01~362.22,q=3.56~43.24,P〈0.01）,术后1年与术后3个月相比各指标均有改善,差异有显著性（q=3.56~8.41,P〈0.05）。结论多普勒超声显像对CHF病人双心室再同步化治疗效果具有较高的评价价值。Objective To investigate the value of Doppler ultrasonic imaging in evaluating the efficacy in patients with chronic heart failure （CHF） receiving biventricular resynchronization therapy （BRT）. Methods Thirty-four patients with CHF complicating left bundle branch block who received BRT were enrolled in the present study. Using Doppler echocardiography, their LAD, LVEDD, LVESD, LVFS, LVEF and SPWMD, LVFT/R-R interval were measured before surgery, 1 and 3 months, as well as 1 year after surgery. Results After 1-and 3-month as well as 1-year BRT therapy, each above parameters improved dra matically compared with before surgery, the differences being significant （F = 19.01 -- 362.22, q = 3.56 - 43.24, P〈0.01 ）. The pa- rameters measured 1 year after the implantation were significantly improved compared with that of 3 months after implantation （q=3.56--8.41 ,P〈0.05）. Canelusion Doppler ultrasonic imaging has a higher value in evaluating the efficacy of biventricular resynchronization therapy for chronic heart failure.
