摘 要:目的探讨硫酸镁对不同孕周子痫前期病人胎儿血流的影响。方法 2012年4月—2013年10月,本院诊治子痫前期病人66例,根据孕周分为Ⅰ组(≤30周)28例,Ⅱ组(>30周)38例。采用超声多普勒测定硫酸镁应用前及应用后8h子宫动脉(UtA)、脐动脉(UA)及胎儿大脑中动脉(MCA)血流阻力指数(RI)、搏动指数(PI)及收缩期与舒张期最大血流速度比值(S/D),同时测定硫酸镁应用前后血清镁及钙水平。结果Ⅰ组病人应用硫酸镁后UtA的RI、PI、S/D及UA、MCA的RI与应用前比较,差异均有显著性(t=3.64~5.34,P<0.01),UA、MCA的PI、S/D差异无显著性(P>0.05)。Ⅱ组病人应用硫酸镁后UtA、UA及MCA的RI、PI、S/D与应用前比较,差异均有显著性(t=2.12~5.90,P<0.01)。66例病人应用硫酸镁后血清Mg水平明显升高,Ca/Mg比值明显降低(t′=16.41、29.20,P<0.01)。应用硫酸镁前后血清Mg水平、Ca/Mg比值与各血流参数均无相关性(P>0.05)。结论应用硫酸镁可影响母体及胎儿血流,其影响效果与血清镁水平无关。Objective To study the effect of magnesium sulfate on fetal blood flow in preeclampsia (PCS). Methods A total of 66 women with PCS treated in our hospital-during March 2012 October 2013 were divided into group 1 (n =28, 430 weeks) and group 2 (n = 38, 30 weeks) according to gestational weeks. Before and eight hours after using magnesium sulfate, resistance index (RI) of uterine artery (UtA), umbilical artery (UA) and fetal middle cerebral artery (MCA), pulsatility index (PI), S/D ratio (S/D) were measured by ultrasonic Doppler, the serum magnesium and calcium were determined as well. Re- sults After medication of magnesium sulfate, the differences of the RI, PI, S/D in UtA in group 1 were significant as compared with that before the medication (t =3.64--5.34,P〈0.01), and that of PI and S/D in UA and MCA were not (P^0.05). In group 2, the differences of RI, PI and S/D in UtA, UA and MCA between before and after medication of magnesium sulfate were signifi- cant (t =2.12--5.90,P^0.01). After serum magnesium therapy of the 66 patients, their serum Mg markedly increased, and the Ca/Mg decreased (t'= 16.41,29.20 P〈0.01). There was no correlation between levels of serum magnesium or Ca/Mg ratio in the two groups between before and after medication of magnesium sulfate (P〉0.05). Conclusion Magnesium sulfate may affect the maternal and fetal blood flow, but the influence is not associated with the serum level of magnesium.
