体质量指数对稳定期COPD病人心肺运动耐力影响

摘　　要：目的 探讨体质量指数（BMI）对慢性阻塞性肺疾病（COPD）病人运动耐力的影响。方法 以COPD稳定期病人126例为研究对象,按照BMI高低分成BMI超标组46例、正常BMI组39例和低BMI组41例,比较各组病人用力肺活量（FVC）占预计值%、第1秒用力肺活量（FEV1）占预计值%和深吸气量占预计值%以及心肺运动试验指标的变化情况,并分析影响心肺运动试验指标的因素。结果 BMI超标组、正常BMI组和低BMI组病人心肺运动试验前FVC、FEV1和深吸气量占预计值%差异无显著性（P〉0.05）。BMI与峰值运动状态时的氧耗量、运动负荷和氧脉搏呈正相关（r=0.547-0.808,P〈0.05）,与呼吸困难指数呈负相关（r=-0.462,P〈0.05）。多因素方差分析显示,BMI是影响病人心肺运动试验指标改变因素（F=34.711,P〈0.01）。结论 COPD病人运动耐力和肌肉做功能力随BMI降低而降低,适当提高BMI,将有助于改善COPD病人的肺功能和运动耐力。Objective To investigate the effect of body mass index （BMI） on exercise tolerance in patients with chronic obstructive pulmonary disease （COPD）. Methods Stable phase 126 COPD patients were enrolled in this study and divided into three groups according to BMI as follows： overweight group （n=46）, normal-weight group （n=41） and low-weight group （n = 41）. The prediction rate of forced vital capcacity （FVC）%, forced expiratory volume in the first second （FEV1）% and inspiratory capacity （IC） % as well as the changes of markers of cardiopulmonary exercise test （CPET） were compared between the three groups and the influence factors analyzed. Results Before CPET, the differences of prediction values of FVC%, FEV1% and IC% between the three groups were not significant （P〉0.05）. At peak exercise state, BMI was positively correlated with oxygen consumption and oxygen pulse （r=0.547-0.808,P〈0.05）, and negatively correlated with dyspnea index （r= -0.462 ,P〈0.05）. Multivariate analysis showed that BMI was the only factor that affected the changes of index of CPET in this cohort study. Conelusion The exercise tolerance and muscle power capability in patients with chronic obstructive pulmonary disease decrease along with the decrease of BMI. Appropriately boost BMI will improve the patient＇s lung function and exercise tolerance.
