双阑尾畸形并阑尾穿孔1例  

Acute perforated appendicitis in a duplicated appendix:a case report

摘　　要：患者,男,49岁。因右下腹持续性疼痛3d入院。患者于3d前突发右下腹部疼痛,无发热,无恶心、呕吐等不适,于外院行抗炎、对症治疗后,腹痛症状无明显好转,遂来我院,门诊下腹部CT平扫提示：阑尾显示不清,阑尾及其周围炎,可疑阑尾穿孔。Summary Double appendix is extremely rare and fewer than 100 cases have been reported in the literature to date. We present a case of operative treatment of acute perforated appendicitis in a doubled vermiform appendix. A 49-year-old male was hospitalized with persistent right lower quadrant pain over the past 3 days,lower abdominal CT scans revealed appendicitis and periappendicular inflammation, suspected appendiceal perforation. As soon as the diagnosis of acute perforated appendicitis was established urgent laparotomy. Urgent laparotomy showed a type A duplicated appendix accompanied by broad thickening mesoappendix. A second appendix was detected with the broad mesoappendix and obvious signs of inflammation, retrocecally, 2 cm from the first one. Formal appendectomy was then performed for double appendix. The patient was cured and discharged on the 20 th postoperative day.
