摘  要:目的初步探讨循环microRNA-126(miR-126)在急性心肌梗死(AMI)患者中早期诊断及预后评估的价值。方法收集175例AMI患者和24名健康人的血浆标本,通过定量反转录聚合酶链反应(qRT-PCR)检测血浆中miR-126的相对含量。结果AMI患者血浆中miR-126的相对含量较健康人降低(P〈0.001),且其在AMI患者中的相对含量与CK-MBmass、Myo及cTnI浓度均呈负相关,相关系数分别为-0.206、-0.219及-0.193(P均〈0.01);miR-126的ROC曲线下面积为0.735(95%C1:0.646-0.825),其诊断AMI的灵敏度为54%,特异性为88%;Kaplan-Meier生存曲线分析发现血浆循环miR-126水平与患者1、6、12,24个月主要不良心脏事件(MACE)及死亡率无关(P=0.202)。结论循环miR-126可作为AMI早期诊断生物标志物的补充,不能作为AMI患者短、长期预后评估的生物学标志物。Objective To evaluate the values of plasma microRNA-126 on diagnosis and prognosis for acute myocardial infarction (AMI) patients. Methods Plasma samples were collected from 175 AMI patients and 24 healthy people, by quantitative reverse transcription polymerase chain reaction (qRT-PCR) assay in plasma by miR-126. The prognostic factors of AMI patients were analyzed by COX proportional hazards models. Kaplan-Meier survival analysis was used to compare survival curves between the two groups. Results The content of miR-126 in the plasma of AMI patients decreased comparexl to healthy people (P〈0.001), and the relative content and CK-MB mass, Myo and cTnI concentrations were negatively correlated, the correlation coefficients were -0.206, -0.219 and -0.193 (all P〈0.01); receiver operating characteristic curve (ROC) analysis showed that, miR-126 in the area under the ROC curve was 0.735 (95% CI: 0.646-0.825), the sensitivity of the diagnosis of AMI was 54%, specificity of 88%. Kaplan-Meier survival curve analysis of circulating plasma miR-126 levels in patients with 1, 6, 12, 24 months major adverse cardiac events and the relationship between the incidence of death, the results found no statistical significance (P=0.202). Conclusion Circulating miR-126 can be used as a supplement biomarker for AMI early diagnosis, and cannot be as a biological marker of short and long-term outcomes for AMI patients.
