干燥综合征肺功能变化特点及其与T细胞亚群CD3、CD4、CD8、BTLA的相关性分析

摘　　要：目的:观察干燥综合征患者肺功能参数与T细胞亚群CD3、CD4、CD8与B、T淋巴细胞衰减因子(BTLA)的变化,探讨干燥综合征患者肺功能变化与T、B细胞的关系。方法:选取干燥综合征患者120例设为观察组,同时选取健康体检者60例设为对照组。采用肺功能仪检测干燥综合征患者肺功能,流式细胞术检测干燥综合征患者外周血CD3、CD4、CD8表面BTLA表达,观察干燥综合征患者实验室指标的变化。结果:干燥综合征患者肺功能降低率为77.5%;与对照组比较,观察组肺功能参数、CD3+BTLA+T、CD4+BTLA+T、CD8+BTLA+T细胞降低(P<0.05或P<0.01);相关性分析显示,肺功能参数FEV1/FVC与PLT、体倦乏力积分呈负相关,MEF25-75与病程、少气懒言积分呈负相关,MEF50与腮腺肿大积分呈负相关,PEF与病程呈负相关,DLCO与病程、超敏C-反应蛋白、口干咽燥积分、面色萎黄积分呈负相关(P<0.05或P<0.01)。FEV1/FVC与Ig G呈正相关,PEF与泪膜破裂时间、CD8+BTLA+T cell呈负相关,DLCO与唾液流率、CD4+BTLA+T cell呈正相关(P<0.05或P<0.01)。结论:干燥综合征患者存在肺功能下降,且肺功能降低与CD3、CD4、CD8等T细胞亚群、BTLA表达失衡有关。Objective：To observe changes of pulmonary function,T cell subsets of CD3, CD4,CD8 and BTLA in patients with sj6gren＇s syndrome and explore their relationship.Methods： 120 cases of sj6gren＇s syndrome were selected as an observation group and another 60 healthy people were selected as a control group.An instrument for pulmonary function was used to detect the pulmonary function of patients with sjogren＇s syndrome,and the Flow Cytometry was used to detect the expression of BTLA on the surface of peripheral blood CD3,CD4 and CD8,observing the changes of laboratory indexes.Results：The decrease rate of pulmonary function was 77.5% in patients with sjogren＇s syndrome.Compared with the control group,the pulmonary function parameters and CD3^＋ BTLA^＋ T, CD4^＋ BTLA^＋ T and CD8^＋ BTLA^＋ T cells in the observation group decreased （ P 〈 0.05 or P 〈 0.01 ） .The correlation analysis showed that pulmonary function parameter FEV1/FVC was negatively correlated with PLT and the fatigue score;MEF25-75 was negatively correlated with the scores of the course of disease and lack of qi and no desire to speak;MEF50 was negatively correlated with the score of parotid gland swelling;PEF was negatively correlated with the course of disease;and DLCO was negatively correlated with scores of the course of disease,high sensitive C-reactive protein,dry mouth and throat, and sallow complexion （ P 〈 0.05 or P 〈 0.01 ） .The FEV1/FVC was positively correlated with IgG;PEF was negatively correlated with break-up time of tear film and CD8^＋BTLA^＋ T cell;and DLCO was positively asso- ciated with salivary flow rate and CD4^＋ BTLA^＋ T cell （ P 〈 0.05 or P 〈 0.01 ） .Conelusion：There is a decline of pulmonary function in patients with sjogren＇s syndrome,which is related with T cell subsets of CD3,CD4 and CD8 and the imbalance of BTLA expression.
