复方利多卡因乳膏在儿童腰椎穿刺中的应用

摘　　要：目的探讨复方利多卡因乳膏在儿童腰椎穿刺中的应用效果。方法选取我科需要进行腰椎穿刺术的150例疑似脑膜炎患儿,将其随机分为观察组和对照组各75例。行腰椎穿刺术前,观察组患儿穿刺部位局部涂抹复方利多卡因乳膏麻醉;对照组患儿应用利多卡因注射液局部分层浸润麻醉。3岁以下儿童采用Flace疼痛评估量表,3岁以上儿童应用Wong-Baker面部表情分级量表对患儿疼痛程度进行评价,并观察腰椎腰椎穿刺成功率。结果观察组患儿疼痛比例为80.0%,明显低于对照组的93.3%,差异显著具有统计学意义(P<0.05)。观察组腰穿成功率为97.3%,对照组为96.0%,两组比较差异不明显,无统计学意义(P>0.05)。结论复方利多卡因乳膏能够显著降低腰椎穿刺术穿刺疼痛,使用简单方便,患者痛苦小,值得临床推广应用。Objective To explore the application effect of compound lidocaine cream on lumbar puncture in children. Methods 150 children with suspected meningitis needed to receive lumbar puncture in our department were selected and randomly divided into observation group and control group, with 75 cases in each group. Before the operation of lumbar puncture, children in observation group were given topical application of compound lidocaine cream, while children in control group received lidocaine injection to give a local infiltration anesthesia. Children under three years old adopted Flace pain assessment scale and children beyond three years old adopted Wong-Baker facial expression classification scale to evaluate the degree of pain, the lumbar puncture success rate of two groups was observed. Results The pain rate of the observation group was 80.0%, significantly lower than 93.3% of the control group, with statistical difference （P 〈0.05）. The lumbar puncture success rate of the observation group （97.3%） and control group （96.0%） had no statistical difference （P〉0.05）. Conclusions Compound lidocaine cream can significantly reduce the stinging pain of lumbar puncture. It is simple and convenient to operate and valuable to promote in clinic.
