乌司他丁与奥曲肽联合治疗重症急性胰腺炎的疗效观察

摘　　要：目的:探讨乌司他丁与奥曲肽联合治疗重症急性胰腺炎(SAP)的疗效。方法:125例SAP患者按随机数字表法分为对照组(n=62)和观察组(n=63)。对照组患者单用奥曲肽治疗,观察组患者采用乌司他丁、奥曲肽联合治疗,对比2组的总有效率、临床症状和体征改善时间、血及尿淀粉酶阳性率、C反应蛋白(CRP)及炎症细胞因子水平、不良反应等指标。结果:治疗后,观察组患者腹痛缓解时间、胰腺水肿消退时间、住院时间明显短于对照组(P<0.05);观察组患者总有效率显著高于对照组(P<0.05);观察组患者血尿淀粉酶阳性率明显低于对照组(P<0.05);观察组患者CRP、白细胞介素-10(IL-10)、白细胞介素(IL-15)和肿瘤坏死因子(TNF-α)等指标明显优于对照组(P<0.05);2组患者均未出现不良反应。结论:乌司他丁和奥曲肽联合治疗SAP的效果显著。OBJECTIVE：To evaluate the efficacy of ulinastatin plus octreotide for severe acute pancreatitis （ SAP） .METHODS：A total of 125 SAP patients were randomly divided into control group （ n=62 ） and observation group （ n=63 ） with control group treated with octreotide and the observation group with ulinastatin plus octreotide. The total effective rate, time to relief of clinical symptoms, positive rates of serum amylase and urine amylase, CRP and inflammatory factors and adverse drug reaction were compared between the two groups. RESULTS： After treatment, the time to relief of abdominal pain, subsiding time of pancreatic edema and the time to discharge were significantly less in the observation group than in the control group （ P 〈0.05 ）; the total effective rate in the observation group was significantly higher than in the control group （ P〈0.05 ） and the positive rates of serum amylase and urine amylase in the observation group were significantly lower than in the control group （P〈0.05）;furthermore, the indicators such as CRP, IL-10, IL-15 and TNF-αin the observation group were all better than in the control group （P〈0.05）.Adverse drug reaction was noted in neither group.CONCLUSIONS： Ulinastatin plus octreotide showed remarkable efficacy for severe acute pancreatitis, which thus is worthy of popularization in clinical application.
