摘  要:目的 探讨并分析荧光定量聚合酶链反应(FQ-PCR)法检测尿巨细胞病毒(CMV)在婴儿CMV感染的诊断与治疗动态监测中的应用价值.方法 以120例确诊或疑似CMV感染观察组患儿(观察组)和120例健康对照组儿童为研究对象,采用荧光定量聚合酶链反应(FQ-PCR)对尿CMV-DNA进行检测,对血细胞pp65抗原及血清CMV-IgM同时进行检测,并且对检测结果进行比较分析.另外,对30例确诊CMV感染患儿给予更昔洛韦治疗后尿CMV-DNA拷贝进行动监测.结果 FQ-PCR检测的灵敏性和特异性均高于pp65抗原检测,两种方法的灵敏性比较有统计学意义(x2=8.004,P<0.05),特异性比较有统计学差异(x2=10.047,P<0.05);FQ-PCR法的灵敏性、特异性均较CMV-IgM法高,两法的灵敏性比较有统计学意义(x2=14.468,P<0.05),特异性比较有统计学意义(x2=37.604,P<0.05).30例确诊患儿尿CMV-DNA病毒拷贝数治疗前后有统计学差异(x2=10.55,P<0.05).结论 FQ-PCR法检测尿CMV在婴儿CMV感染的诊断与治疗动态监测中有良好的应用价值,值得临床推广.Objective To investigate and analyze the fluorescence quantitative polymerase chain reaction (FQ-PCR) method to detect urine cytomegalovirus (CMV) application value in the diagnosis and treatment of CMV infection in infants in the dynamic monitoring.Methods 120 patients with confirmed or suspected CMV were collected as observation group,and 120 healthy children were collected as controll group.Urine CMV-DNA was detected using FQ-PCR assay,and blood cell antigen pp65 and serum CMV-IgM were tested at the same time.Results The sensitivity and specificity of FQ-PCR were higher than that of pp65 and that of CMV-IgM.(x2 =10.55,P =0.000 〈 0.05).Conclusions The detection of urinary CMV FQ-PCR method has good application value in the diagnosis and treatment of CMV infection in infants in dynamic monitoring,which is worth the clinical promotion.
