风心病瓣膜置换术前后肺淤血X线表现变化分析

摘　　要：目的分析126例风湿性心脏瓣膜病患者手术前后肺淤血X线变化及相关因素。方法对126例风湿性心脏瓣膜病患者的临床资料和X线表现进行回顾性分析并采用SPSS 13.0统计软件进行数据处理。结果 62例二尖瓣置换病例和64例联合瓣膜置换病例(二尖瓣置换+主动脉瓣置换),按术后复查的X线表现分成2组进行统计分析,计算所得χ2值为6.13,P<0.05。结果表明,两组患者肺淤血愈合率不相同,单纯二尖瓣病变患者术后肺淤血愈合率高于联合瓣膜病变患者术后肺淤血愈合率。结论风湿性心脏病瓣膜置换术后半个月,单纯二尖瓣病变术后肺淤血有明显改变。部分患者肺淤血未减轻主要考虑为术前病史长,心功能差,以及早期并发症的影响,还有远期遗留左心房增大、肺动脉高压、风湿活动、服药不规律等因素。Objective To analyze pulmonary congestion X-ray changes of 126 cases of rheumatic heart valve disease before and after operation and the related factors. Methods The clinical datas of 126 cases of rheumatic heart valve disease and X-ray manifestation were retrospectively analyzed using SPSS 13.0 statistical software for data processing. Results 62 cases mitral valve replacement and 64 cases combined valve replacement（mitral valve replacement and aortic valve replacement） ,according to the X-ray manifestation of postoperative review was divided into two groups through statistical analysis,calculate the χ2 value of 6.13,P〈0.05）. The results show that the two groups of patients with pulmonary congestion healing rate of different pulmonary congestion postoperatively in patients with pure mitral valve lesions healing rate was higher than combined valve disease in patients with postoperative pulmonary congestion healing rate. Conclusion Rheumatic heart valve replacement after half month,pure mitral valve lesions of postoperative pulmonary congestion have obvious change. Part of the patients with pulmonary congestion did not reduce the main consideration for preoperative history of long and poor cardiac function,and the influence of early complications, and long-term legacy left atrial enlargement,pulmonary hypertension,rheumatism,irregular medication and other factors.
