摘  要:目的探讨血液灌流(hemoperfusion,HP)联合连续性静-静脉血液滤过(continuous venovenous hemofiltration,CVVH)对百草枯中毒的治疗效果。方法将我院急诊科收治的百草枯中毒120例根据治疗方法分为观察组53例和对照组67例,两组均予常规及HP治疗,在此基础上观察组予CVVH治疗。治疗1个月后观察两组总有效率、死亡病例存活时间、急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)及多器官功能障碍综合征(multiple organ dysfunction syndrome,MODS)的发生率。结果观察组、对照组总有效率分别为92.45%、73.13%,组间比较差异有统计学意义(P<0.05);与对照组比较,观察组死亡病例存活时间延长,ARDS、MODS发生率降低,差异均有统计学意义(P均<0.05)。结论 HP联合CVVH治疗百草枯中毒可有效提高抢救成功率,降低并发症发生率。Objective To evaluate the treatment outcome of hemoperfusion with continuous venovenous hemofiltration ( CVVH) for paraquat ( PQ) poisoning. Methods 120 patients with acute PQ intoxication were included in this study. All the patients were divided into two groups according to different treatment methods. All the patients received routine treatment, among whom, 53 patients received hemoperfusion with CVVH ( observation group) and 67 patients received hemoperfuion ( control group) . Statistical method was used to investigate the differences, including cure rate, mortality rate, incidence rate of acute respiratory distress syndrome ( ARDS) and multiple organ dysfunction syndrome ( MODS) between the two groups af-ter 1 month of treatment. Results Effect rates of observation group and control group were 92. 45% and 73. 13% respective-ly. There was statistical significance between the two groups (P〈0. 05). The survival time and the incidence rates of ARDS and MODS were significant decreased in observation group compared with that of control group (P〈0. 05). Conclusion He-moperfusion with CVVH can improve the rescue success rate and reduce complication incidence rate.
