高压氧联合天麻素及甲钴胺治疗糖尿病周围神经病变的临床观察

摘　　要：目的观察高压氧联合天麻素及甲钴胺治疗糖尿病周围神经病变的临床效果。方法收集2型糖尿病周围神经病变患者72例,随机分为2组,每组36例。患者均予降糖药物口服或胰岛素皮下注射降血糖治疗,对照组给予天麻素6 ml静脉滴注及甲钴胺500μg静脉滴注,治疗组在对照组基础上加用高压氧(HBO)治疗,压力0.2MPa,吸氧30 min换吸空气5 min,反复2次,总吸氧时间60 min,1次/d。2组均治疗2周,比较治疗后的临床效果、感觉神经传导速度(SNCV)和运动神经传导速度(MNCV)。结果治疗组的临床治疗效果明显优于对照组(86.2%vs.63.9%),差异有统计学意义(P<0.05)。与治疗前比较,治疗组腓总神经、胫后神经的SNCV、MNCV及腓肠神经的SNCV均明显增快(P<0.01);对照组腓总神经MNCV明显增快(P<0.01),其他神经传导速度无明显变化(P>0.05)。治疗后2组间比较,治疗组腓总神经、胫后神经、腓肠神经的SNCV均较对照组增快(P<0.01),而MNCV差异无统计学意义(P>0.05)。结论高压氧联合天麻素及甲钴胺治疗糖尿病周围神经病变有较好的效果,值得临床应用。Objective To investigate the effect of hyperbaric oxygen combined with gastrodin and mecobalamin on diabetic peripheral neuropathy.Methods Collected type 2 diabetic patients with peripheral neuropathy in 72 cases,they were randomly divided into 2 groups,36 cases in each group.All patients were treated with oral antidiabetic drugs or subcutaneous injection of insulin hypoglycemic therapy,the control group was given gastrodin intravenous infusion of 6 ml and mecobalamin500 μg intravenous infusion,the treatment group besides the same as control group,also received hyperbaric oxygen（HBO）therapy with oxygen pressure of 0.2 MPa,30 min with 5 min inhalation of air,repeated 2 times,the total oxygen time 60 min,once a day.The 2 groups were treated for 2 weeks,the clinical effect after treatment,the sensory nerve conduction velocity（SNCV） and motor nerve conduction velocity（MNCV） were compared.Results The clinical therapeutic effect of the treatment group was significantly better than the control group（86.2%vs.63.9%）,the difference was statistically significant（P 0.05）.Compared with before treatment,the treatment group of common peroneal nerve,tibial nerve＇ s SNCV and MNCV and sural nerve＇ s SNCV were significantly increased（P〈0.01）;the control group＇ s common peroneal nerve＇ s MNCV increased obviously（P〈0.01）,other nerve conduction velocity revealed no significant changes（P〉 0.05）.After treatment,between the 2 groups,the treatment group common peroneal nerve and posterior tibial nerve and sural nerve＇ s SNCV were lower than those in the control group increased（P〈0.01）,but no significant difference was found in MNCV（P 0.05）.Conclusion Hyperbaric oxygen combined with gastrodin and mecobalamin revealed good effect on diabetic peripheral neuropathy,its worthy of clinical application.
