芪苈强心胶囊对慢性肾衰竭合并慢性收缩性心力衰竭患者心功能的影响

摘　　要：目的观察芪苈强心胶囊对慢性肾衰竭合并慢性收缩性心力衰竭患者血浆脑钠肽(BNP)、C反应蛋白(CRP)及心功能的影响。方法80例慢性肾衰竭合并慢性收缩性心力衰竭患者随机分为对照组、研究组各40例。2组患者均常规应用利尿剂、血管紧张素转换酶抑制剂、β受体阻滞剂、硝酸酯类、洋地黄等药物治疗,研究组在对照组治疗基础上加服芪苈强心胶囊4粒,3次/d。观察治疗前及治疗8周后2组患者血浆BNP、CRP表达水平及左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)的变化。结果2组有效率比较,研究组总有效率明显高于对照组(B2.5%vs.65.0%,P<0.05)。治疗前2组患者血浆BNP、CRP水平及LVEDV、LVESV、LVEDD、LVEF比较差异均无统计学意义(P>0.05)。与治疗前比较,2组患者治疗8周后血浆BNP、CRP水平均降低(P<0.05),LVEDV、LVESV、LVEDD、LVEF均得到有效改善(P<0.05)。与对照组比较,研究组血浆BNP、CRP水平明显降低(P<0.05),心功能显著改善(P<0.05)。结论芪苈强心胶囊能明显降低慢性肾衰竭合并慢性收缩性心力衰竭患者血浆BNP、CRP表达水平,改善患者心功能,提高临床治疗效果。Objective To observe the effect of Qiliqiangxin capsule on plasma brain natriuretic peptide（BNP）,Creactive protein（CRP） and cardiac function in patients with chronic renal failure and chronic congestive heart failure.Methods Eighty patients with chronic renal failure and chronic congestive heart failure were randomly divided into control group and study group with 40 cases in each group.2 groups were treated with routine application of diuretics,angiotensin-converting enzyme inhibitors,beta blockers,nitrates,digitalis drug treatment,the study group also received Qiliqiangxin capsule 4 grain,3 times /d.Before and 8 weeks after treatment,plasma BNP and the expression level of CRP and left ventricular end diastolic volume（LVEDV）,left ventricular end systolic volume（LVESV）,left ventricular end diastolic diameter（LVEDD）,left ventricular ejection fraction（LVEF） changes were observed and compared between the 2 groups.Results The effective rate in the study group was belter than that in control group（82.5%vs.65.0%,P〈0.05）.Before treatment,plasma BNP,CRP level and LVEDV,LVESV,LVEDD,LVEF in the 2 groups revealed no significant differences（P〈0.05）.Compared with before treatment,plasma BNP and CRP levels after 8 weeks were decreased in both of the two groups（P〈0.05）,LVEDV,LVESV,LVEDD,LVEF were improved（P〈0.05）.Compared with the control group,levels of serum BNP,CRP were decreased significantly（P〈0.05） and the heart function improved significantly in the study group（P〈0.05）.Conclusion It demonstrated that the Qiliqiangxin capsule could significantly decrease the expression level of plasma BNP and CRP in patients with the chronic renal failure and chronic congestive heart failure;it can improve the heart function and the clinical treatment effect.
