摘  要:目的研究外用重组人粒细胞-巨噬细胞集落刺激因子(rh GM-CSF)对深Ⅱ度烫伤创面愈合的影响。方法90只Wistar大鼠建立深Ⅱ度烫伤模型,分为A、B、C三组,每组30只。A组:凡士林纱布覆盖;B组:纳米银敷料覆盖;C组:rh GM-CSF涂抹创面。伤后第1,4,7,10,14,21天,观察创面病理学改变,按照酶联免疫吸附法测定血清中表皮生长因子(EGF)水平,运用逆转录-聚合酶链反应(RT-PCR)检测创面愈合过程中转化生长因子(TGF)-β1mRNA表达的变化。结果病理学改变:A、B、C三组均在第10天出现明显的上皮化;EGF水平逐渐上升,A组上升最慢,C组上升最快,第1,4天各组间及第7天A组与B组差异无统计学意义(P>0.05),第7天其余各组间及第10,14,21天各组间差异均有统计学意义(均P<0.05);TGF-β1mRNA相对表达量,A组、B组逐渐上升,C组在第14天达到峰值,第21天下降,第1天,B组与C组之间差异有统计学意义(P<0.05),第4天各组间差异无统计学意义(P>0.05),第7,10,14,21天各组间差异有统计学意义(P<0.05)。结论 rh GM-CSF可加速深Ⅱ度烫伤创面愈合。Objective To study the effect of recombinant human granulocyte/macrophage colony stimulating factor (rhGM-CSF) on wound healing in rat model of deep second-degree burn. Methods The burn model was established with Wistar rats. They were randomly divided into three groups, petrolatum treatment group (group A, n= 30) , nano-silver treatment group ( group B, n = 30), and rhGM-CSF treatment group ( group C, n = 30). Pathological changes of wound of the three groups were observed on the 1st, 4th, 7th, 10th, 14th, and 21st day 'after treatment. Meanwhile the concentration of epidermal growlh factor (EGF) in serum was measured with ELISA, and TGF-13t mRNA expression was detected by RT-PCR. Results Epithelization was observed in all the groups on the 10th day after treatment. EGF level increased gradually, and it increased most slowly in group A, and fastest in group C. There was no significant difference in EGF level among the groups on the I st and 4th days, and between group A and B on the 7th day (P〉0.05). But there was significant difference in in EGF level hetween group A and C, between group B and C on day 7, and among the groups on day 10, 14 and 21, respectively (P〈0.05). Relative expression of TGF-β mRNA was gradually increased in group A and group B, and peaked on day 14 and decreased on day 21 in group C. There was significant difference between group B and group C on day 1 ( P〈0.05 ) , no significant dift)rence on day 4 (P〉0.05) and. significant difference among the groups on day 7, 10, 14 and 21 (P〈0.05). Conclusion rhGM-CSF treatment can accelerate healing of burn wound.
