摘  要:目的分析HIV抗体不确定的血清学特征及预示HIV感染的意义,评价病毒载量鉴别HIV抗体不确定的效果。方法收集18例HIV抗体不确定标本,分析免疫印迹确认的条带类型,进行病毒载量检测,以随访的结果为金标准。结果 18例不确定标本共有6种带型,env类不确定的构成比例最多。18例接受随访的HIV抗体不确定病例中,9例发生HIV抗体阳转,大多数为env,阳转病例的HIV抗体发展很快,约2周就可以看出变化。病毒载量检测鉴别HIV抗体不确定的敏感性好。结论针对gag蛋白的不确定反应较常见,但大都是非特异反应。env类不确定预示HIV感染的意义较大。HIV抗体不确定如为早期HIV感染,血清抗体发展的速度很快,大约2周抗体就有发展。病毒载量检测可有效鉴别不确定结果。Objective To study the indeterminant serological characteristics of indeterminate HIV antibody and to evaluate the indeterminant efficiency of HIV viral load test to discriminate HIV antibody. Methods 18 HIV antibody indeterminant serum samples were collected,the western blot pattern and HIV viral load test were analyzed,and regarded the random visit results as the gold standards. Results There were 6 patterns among 18 indeterminant HIV antibody,the proportions of env were most.9 HIV antibody indeterminant cases were converted to HIV antibody positive among 18 followed individuals. They were most env indeterminant and HIV antibody developed rapidly. HIV viral load was an ideal assay to discriminate HIV antibody indeterminant with best sensitivity. Conclusion The indeterminant of gag were common,but were unspecific reaction. Env indeterminant were with the greatest predictive value of HIV infection. If HIV antibody indeterminant was in the early HIV infection,the serum antibody grew rapidly,and the antibody could get developed in about two weeks. Viral load assay can be applied to discriminate HIV antibody indeterminant results.
