超声与核素显像联合应用对甲状腺结节诊断价值的评估

摘　　要：目的分析超声、核素显像及两者联合应用对甲状腺结节的诊断价值。方法收集2010年1月至2012年12月在内蒙古医科大学附属医院行手术治疗的甲状腺结节患者500例,共768个甲状腺结节,患者分别行99mTcO4-核素显像、超声检查,对结果进行对比研究。结果 768个甲状腺结节术后病理诊断良性结节698个,甲状腺癌70个。(1)超声诊断:70个恶性结节超声诊断为真阳性48个,假阴性22个,灵敏度68.5%;698个良性结节超声诊断为假阳性182个,真阴性516个,特异性73.9%。(2)核素诊断结果:70个恶性结节核素诊断为真阳性44个,假阴性26个,灵敏度62.8%;698个良性结节核素诊断为假阳性242个,真阴性456个,特异性65.3%。(3)核素与超声联合诊断结果:70个恶性结节超声诊断为真阳性61个,假阴性9个,灵敏度82.8%;698个良性结节超声诊断为假阳性276个,真阴性422个,特异性60.5%。(4)经χ2检验,超声和两者联合应用之间的灵敏度比较,χ2=7.002,P=0.008<0.05;特异度,χ2=28.713,P=0.000<0.05;核素显像和两者联合应用之间的灵敏度比较,χ2=11.010,P=0.001<0.05;特异度,χ2=3.548,P=0.060>0.05。结论 99mTcO4-核素显像联合超声检查的灵敏度高于单独应用,因此,两者联合应用有助于评价甲状腺结节的功能及性质,对鉴别诊断甲状腺结节良恶性起着重要作用。Objective To analyze the value of combined imaging in diagnosis of thyroid nodules through the comparison of ultrasound, radionuclide imaging and the combination of both. Methods Collecting 500 cases of patients with a total of 768 thyroid nodules who were treated with operation from 2010 January to 2012 December in the Affiliated Hospital of Inner Mongolia Medical University. All the patients were performed 99mTcO4 - scintigraphy, ultrasonography examination and the combination of both, and the results were conducted comparative study. Results There were 698 benign thyroid nodules confirmed by postoperative patholog- ical diagnosis, 70 nodules were thyroid carcinoma. （1）The sensitivity and specificity of ultrasonography were 68.5 % and 73.9 % respectively. （2） The sensitivity and specificity of radionuclide imaging were 62, 8 % and 65.3 % respectively. （3）The sensitivity and specificity of combined imaging were 82.8 % and 60.5 % respectively, （4） The results were analyzed by chi - square test. Conclusion The sensitivity of combined application of ul- trasound and radionuclide imaging is higher than that of single application. The combined application is sueful in evaluating and differentiating thyroid nodules.
