摘 要:目的分析2型糖尿病患者血尿酸与尿蛋白之间的关系。方法选择2011年1月至2013年1月我院收治的2型糖尿病患者280例。根据患者尿蛋白排泄率(UAER)和(或)24 h尿蛋白量分为尿蛋白量正常组(A组);微量蛋白尿组(B组);大量蛋白尿组(C组)。分析各组间一般资料、临床表现及血生化检测结果。结果 B组、C组两组患者收缩压、糖尿病病程、血肌酐、血尿酸与A组比较差异有统计学意义(P<0.05),其中血肌酐、血尿酸在C组最高,A组最低(P<0.01);eGFR在C组最低,A组最高(P<0.01)。与A组相比,B组、C组患者总胆固醇、甘油三酯、糖化血红蛋白比率明显升高(P<0.05)。C组、B组患者的高尿酸血症发生率明显高于A组。多元逐步回归分析示血尿酸水平是白蛋白尿的独立危险因素。结论 2型糖尿病患者血尿酸是白蛋白尿的主要影响因素。Objective To analyze the relationship between type 2 diabetic uric acid and urinary protein level. Methods 280 patients with type 2 diabetic mellitus in 2011 January 2013 January selected in ChiFeng Hospital were cases. According to the urinary protein excretion rate(UAER) and(or) 24hour urinary protein quanti- ty divided into normal group(A group), microalbuminuria group (B group) and mass albuminuria group (C group). Analyzed general data, clinical manifestation and blood biochemical detection results. Results Systolic blood pressure, diabetes, creatinine, uric acid in B group and C group comparison with A group had significant difference. Serum creatinine, blood uric acid in C group was the highest, lowest in A group; eGFR in C group was the lowest, highest in A group. Compared with A group, total cholesterol, triglyceride, glycosylated hemoglobin in patients in B group and C group were significantly increased. Patients in C group and B group with hyperuricemia incidence was significantly higher than in A group. Multiple stepwise regression analysis showed that uric acid level was independent risk factors of albuminuria. Conclusion Uric acid in patients with type 2 diabetes mellitus uric acid is the main influence factor of albuminuria.
