嗜麦芽窄食单胞菌对复方新诺明耐药的机制研究

摘  要:目的:检测 I类整合酶基因(intI 1)及 qacE△1-sul1和 sul2基因在耐复方新诺明的嗜麦芽窄食单胞菌中的分布情况,并探讨基因分布与细菌耐药性的关系。方法收集临床分离的嗜麦芽窄食单胞菌,煮沸法提取总 DNA,采用 PCR法检测 intI 1,qacE△1-sul1和 sul2基因在复方新诺明敏感菌株和耐药菌株中的分布情况。结果28株耐复方新诺明嗜麦芽窄食单胞菌中有25株检出 I类整合酶基因,检出率为89.29%;21株检出 qacE△1-sul1基因,检出率为75%;15株检出sul2基因,检出率为53.57%;18株复方新诺明敏感的嗜麦芽窄食单胞菌中有5株检出 I 类整合酶基因,检出率为27.78%,4株检出 qacE△1-sul1基因,检出率为22.22%,均未检出 sul2基因。结论耐复方新诺明嗜麦芽窄食单胞菌大部分携带有 intI 1,qacE△1-sul1和 sul2基因,嗜麦芽窄食单胞菌对复方新诺明的耐药性与 intI 1,qacE△1-sul1和 sul2的存在有关。Objective Class 1 integrase(intI 1),qacE△1-sul1 and sul2 genes were detected in trimethoprim-sulfamethoxazole resistant Stenotrophomonas maltophilia by PCR to assess the relationship between the antibiotic resistance mechanism for trimethoprim-sulfamethoxazole (TMP-SMZ)and these genes distribution.Methods S.maltophilia isolates were collected from patients treated in Affilitated hospital of Nanjing University of TCM during January to May in 2013,DNA was ab-stracted by boiling method and genes were detected by polymerase chain reaction.Results intI 1 genes were observed posi-tive in 25 of 28 strains resistant for TMP-SMZ,qacE△1-sul1 genes were positive in 21 while sul2 genes positive in 15,the positive rates of intI 1,qacE△1-sul1 and sul2 genes were 89.29%,75% and 53.57%;in 18 trimethoprim-sulfamethoxazole sensitive strains,5 were intI 1 positive,4 were qacE△1-sul1 positive while sul2 were none,the positive rates of intI1 and qacE△1-sul1 were 27.78% and 22.22%.Conclusion Most of stenotrophomonas maltophilia resisted trimethoprim-sulfa-methoxazole had intI 1,qacEΔ1-sul 1 and sul2 genes.
