ROI-CCage治疗单节段脊髓型颈椎病30例早期疗效分析

摘　　要：[背景]探讨ROI-C融合器经前路减压治疗单节段脊髓型颈椎病（CSM）的临床疗效.[病例报告]回顾性分析因诊断为单节段CSM且接受ROI-C Cage治疗并获得回访的30例患者的临床资料,分析手术时间、术中失血量、住院时间及并发症发生情况,并对手术治疗效果进行评价.结果显示,手术时间为（52±8）min,术中失血量为（25.0±5.0）mL,住院时间为（5.0±1.0）d,术后均未发生吞咽困难、伤口血肿、呼吸困难等并发症.随访时间为12~27个月,平均为15.6个月.术后3,6,12个月时的VAS评分、JOA评分、Cobb角及椎间高度较术前均明显改善（P〈0.05）.术后12个月随访结果显示VAS评分优22例、良6例、可2例,优良率为93%（28/30）;JOA评分优20例、良7例、可3例,优良率为90%（27/30）.随访过程中未发现Cage及固定片移位、下沉、断裂,术后6个月时椎间稳定率为100%.[讨论]采用ROI-C Cage治疗单节段CSM可获得良好的减压效果及临床疗效,是目前治疗单节段CSM的较好方法.BACKGROUND To evaluate the clinic curative effects of anterior cervical decompress and implantation of ROI-C cervical intervertebral fusion cage in the treatment of single segmental cervical spondylotic myelopathy（CSM）.CASE REPORTS It was retrospectively analyzed from 2010 to 2014that clinical data,including operation time,intraoperative estimated blood loss,duration of hospitalization and occurrence status of complications,of 30 patients with single segmental CSM treated by ROI-C cervical intervertebral fusion cage.The operation time was 52±8 min,intraoperative estimated blood loss was25.0±5.0mL,and the duration of hospitalization was 5.0±1.0d,and there had no postoperative dysphagia,wound hematoma and dyspnoea,and so on.All patients were followed up for average time of15.8months（12to 27months）.The VAS score,JOA score,Cobb angle and intervetebral height at 3,6and12 months improved significantly after surgery than before surgery（P〈0.05）.The results of following up at 12 months showed that there were excellent in 22 cases,good in 6cases,fair in 2cases and the excellent and good rate in 93%（28/30）according to VAS evaluation standard,and there were excellent in 20 cases,good in 7cases,fair in 3cases and the excellent and good rate in 90%（27/30）according to JOA evaluation standard.No cage shift,sink or breakage had been observed,and the intervertebral stabilization rate was almost 100% at 6month after the surgery.DISCUSSION The ROI-C cervical intervertebral fusion cage has good decompressing and curative effects,and is one of good fusion operation in the treatment of single segmental CSM.
