人乳头状瘤病毒E6/E7mRNA检测筛查宫颈病变的价值

摘　　要：目的评价人乳头状瘤病毒(HPV)E6/E7 mRNA检测在宫颈病变筛查中的价值。方法 2 354例门诊妇女行宫颈液基细胞学(thin-cytologic test,TCT)检查联合HPV E6/E7 mRNA检测,两项检查中任一项或两项阳性的452例妇女行阴道镜下宫颈活检。结果 HPV E6/E7 mRNA表达量随宫颈病变程度加重而增加(χ2=80.366,P=0.000<0.01)。通过ROC曲线分析,以1 484.3 copy/ml为阳性分界点,诊断宫颈高级别病变的敏感度为74.2%,特异度为72.7%。结论HPV E6/E7 mRNA检测在宫颈病变筛查中具有潜在价值,HPV E6/E7 mRNA表达量超过1 484.3 copy/ml的妇女可初步锁定为宫颈癌的高危人群。Objective To investigate the clinical value of human papillomavirus（ HPV） E6 / E7 mRNA detection in the screening of cervical lesions. Methods 2 354 cases of women were enrolled for the detection of thin- cytologic test and HPV E6 / E7 mRNA transcription. 452 cases with positive results of either detection were performed by pathology technique. Results HPV E6 / E7 mRNA expression quantity increased with the aggravating degree of cervical lesions（ χ^2= 80. 366,P = 0. 000 0. 01）.The cut- off point of HPV E6 / E7 mRNA expression quantity for predicting high grade cervical lesions was 1 484. 3 copy / ml,the sensitivity were 74. 2%,specificity were 72. 7%. Conclusion The detection of E6 / E7 mRNA has potential value in the screening of cervical lesions,and women with HPV E6 / E7 mRNA expression quantity more than 1 484. 3 copy / ml can be classified as high- risk group of cervical cancer.
