单纯型和混配型急性有机磷农药中毒患者的血清细胞间粘附分子-1的变化及临床意义

摘　　要：目的:探讨单纯及混配型急性有机磷农药中毒患者血清细胞间粘附分子-1(ICAM-1)的变化及临床意义。方法:将2012年2月至2014年2月收治入院的急性有机磷农药中毒患者为中毒组(n=140)和健康体检者为对照组(n=142),中毒组再分为单纯型组(n=72)、混配型组(n=68),比较对照组及中毒组入院、出院的ICAM-1水平。结果 :中毒组入院时血清ICAM-1平均水平明显高于对照组(815.43±30.58 ng/ml VS 503.34±28.47 ng/ml,P<0.05),出院时与对照组无明显差异(528.23±31.43 ng/ml VS 503.34±28.47 ng/ml,P>0.05);中毒组入院时血清ICAM-1平均水平明显高于出院时(815.43±30.58 ng/ml VS 528.23±31.43 ng/ml,P<0.05);单纯型组和混配型组入院、出院时血清ICAM-1平均水平均无显著性差异(799.53±38.29 ng/ml VS 800.45±30.29 ng/ml,522.75±25.64ng/ml VS 530.37±26.39 ng/ml,P均>0.05),但出院时均明显低于入院时(522.75±25.64ng/ml VS799.53±38.29 ng/ml,530.37±26.39 ng/ml VS 800.45±30.29 ng/ml,P均<0.05)。结论 :单纯型和混配型急性有机磷农药中毒患者的血清ICAM-1水平入院时无显著差异,均可按单纯型急性有机磷农药中毒原则进行救治。Objective：To explore the change and the clinical significance of serum intercellular adhesion molecule-1（ICAM-1） in patients with pure or mixed acute organophosphorus pesticides poisoning（AOPP）. Methods：The patients with AOPP were defined as toxic group（n=140） and the healthy control group（n=142）from Feb 2012 to Feb 2014,and the toxic group were divided into pure group （n=72） and mixed group （n=68）,each group were measured serum ICAM-1 level by ELISA method when they were admitted and leaved hospital. Results：The level of serum ICAM-1 in the toxic group admitted hospital was significantly higher than that in the control group （815.43± 30.58 ng/ml VS 503.34 ±28.47 ng/ml,P〈0.05）,but there was no significantly different when they leaved hospital （528.23±31.43 ng/ml VS 503.34±28.47 ng/ml,P＆gt;0.05）. Although the level of serum ICAM-1 in pure group and mixed group were no significant difference at admitted and leaved hospital （799.53 ±38.29 ng/ml VS 800.45 ±30.29 ng/ml,522.75 ±25.64 ng/ml VS 530.37 ±26.39 ng/ml,both P＆gt;0.05）,but they were significantly lower at leaved hospital than those at admitted hospital （522.75 ±25.64 ng/ml VS 799.53 ±38.29 ng/ml,530.37 ±26.39 ng/ml VS 800.45±30.29 ng/ml,both P〈0.05）. Conclusions：The level of serum ICAM-1 in patients with pure and mixed AOPP is no significant difference at admitted hospital,and the therapeutic principles are according to pure AOPP.
