摘 要:目的:探讨1,3-β-D葡聚糖对早产儿真菌感染的早期诊断及疗效判断的临床价值。方法:选取2012年6月~2014年6月在该院新生儿重症监护病房(NICU)住院、临床怀疑为真菌感染的早产儿56例,进行血培养,同时行血浆1,3-β-D葡聚糖检验,血培养阳性者为观察组,阴性者为对照组,对比两组血浆1,3-β-D葡聚糖含量的差异,并在抗真菌治疗后第2天、第4天及第7天连续检测观察组血浆1,3-β-D葡聚糖含量,评价血浆1,3-β-D葡聚糖在早产儿真菌感染时的早期诊断及疗效。结果:观察组治疗前血浆1,3-β-D葡聚糖含量为(335.3±120.7)pg/ml,对照组为(35.6±22.8)pg/ml,两组比较差异有统计学意义(P〈0.05);观察组抗真菌治疗前与治疗后第2天、第4天及第7天血浆1,3-β-D葡聚糖呈线性下降关系(P〈0.05)。结论:血浆1,3-β-D葡聚糖可指导早产儿真菌感染的早期诊断并评估治疗效果,以减少早产儿真菌感染并发症及耐药菌株率,提高早产儿治愈率及生存质量。Objective: To explore the clinical value and curative effect of 1, 3 - β - D glucan in diagnosis of fungal infection in premature infants. Methods: Fifty -six premature infants suspected of fungal infection were selected from NICU of the hospital from June 2012 to June 2014, blood culture was performed, plasma 1, 3 - β -D glucan test was conducted, the infants with positive blood culture re- sult were designed as observation group, while the infants with negative blood culture result were designed as control group; the concentra- tions of plasma 1, 3 - β - D glucan in the two groups were compared; the concentrations of plasma 1, 3 - β- D glucan in observation group were detected on the second day, the fourth day and the seventh day after antifungal therapy; early diagnosis and curative effect of plasma 1, 3 - β - D glucan in premature Infants with fungal infection were evaluated. Results: The concentrations of plasma l, 3 - β - D glucan in observation group and control group were (335.3 ±120. 7) pg/ml and (35.6 ± 22. 8 ) pg/ml, respectively, there was statistically significant difference between the two groups ( P 〈 0.05 ) ; the concentrations of plasma 1, 3 - β - D glucan in observation group on the second day, the fourth day and the seventh day after antifungal therapy decreased linearly (P 〈 0. 05) . Conclusion : Plasma 1, 3 - β - D glucan can guide early diagnosis and evaluate therapeutic effect of fungal infection, reduce complications of fungal infection and the rate of drug resistant strains and improve the cure rate and quality of life of premature infants.
